Essential Public Health Services Statement Public health nursing practice in Webster County Health Department is defined by the core functions and essential services of public health. The following services are the foundation of all job descriptions: Monitor health problems; Diagnose and investigate health problems health status to identify community and health hazards in the community; Inform, educate, and empower people about health issues; Mobilize community partnerships to identify and solve health problems; Assist in Development of policies and plans that support individual and community health efforts; Enforce laws and regulations that protect health and ensure safety; Link people to needed personal health services and assure the provision of health care when otherwise unavailable; Assure a competent public health and personal health care workforce; Evaluate effectiveness, accessibility, and quality of personal and population-based health services; participate in Research for new insights and innovative solutions to health problems.

Job Summary This position works under the general management of the Executive Director. This position requires coordinating, implementing and monitoring projects and programs within established guidelines for targeted populations and provides professional nursing services to clients in a home or clinic setting. It requires skills to identify health related problems in a variety of settings and implement appropriate intervention measures to promote standard health practices. Participation in unique community health programs to promote overall wellness and community involvement is also required.

Performance Responsibilities Providing nursing services in a wide array of settings and environments which include assessment of client needs; development of health related goals; provision of client care; referral to other agencies, physicians, and health care providers; client record maintenance according to agency protocol; assurance of client safety and security; and reporting observed or suspected child/adult abuse per agency policy. Document client and family health goals in order to provide a systemic health care plan for the improvement of health by reviewing assessment data including client and family interviews; conferencing with medical staff to determine appropriate health care goals and methodology; and communicating regularly with physicians, family members, pharmacists, and other professionals in the provision of service. Submit annual or requested written reports for agency administration and program evaluation by compiling statistics, health record audits, and work activity documentation. Improve community awareness by implementing on-going health service programs that include, but are not limited to blood pressure screening; well elderly and other clinics; school related services and administering immunizations to infants, children and adults including Flu and maintaining accurate records. Perform program services in such areas as Maternal Child & Adolescent Health, WIC; appointment set-up; assisting and processing presumptive Medicaid eligibility applications; health assessment completion; and referrals to needed service and perform accurate client follow up. Provide regular support and supervision to all direct service personnel for which direct supervision is requested. Work directly with community businesses, schools and individual community members to organize and plan for mass vaccination clinics, individual clinics, and clinics located within business, schools, homes, etc. Coordinate public health response, in home health assessments, provide guidance and prevention and mitigation instruction to skilled nursing facilities, assisted living homes, shelters, jails, and prison in addition to our community. Provide accurate and timely record keeping and data entry into agency EHR. Report to Executive Director status of vaccination rates within the county. Assist with education and support to individuals & businesses within the community. Provide Public health surveillance – monitoring the cases, provide testing, trending data, monitoring for variant virus. Provide vaccination programs, outreach to rural communities, medical care for people based within the county that DO NOT require hospitalization. Provide home visits for vulnerable populations and residences in a homebound status or residents that have extreme difficulty leaving their home. Provide a home visit to assess residents, possibly provide O2 for people at home, coordinate meal delivery, etc. and monitor the health of positive cases NOT needing hospitalization. May perform some home health functions when necessary.

Required Qualifications/Experience
An associate degree or diploma in nursing from an accredited nursing program and a minimum of four years experience in nursing and/or A baccalaureate degree in nursing from an accredited college or university and licensure as a registered nurse in Iowa.

Physical Demands: Regularly walk, stand, or stoop; frequently lift, carry, push , pull, or otherwise move objects weighing up to 50 pounds; Use tools or equipment requiring a high degree of dexterity; Regularly drive a motor vehicle; and work for sustained periods of time maintaining concentrated attention to detail. Accommodation may be made for some of these physical demands for otherwise qualified individuals who require and request such accommodation.

Environmental Conditions: Work in an environmentally controlled building; Work exposes incumbent to communicable diseases and environmental hazards; Work requires use of protective devices such as masks, goggles and gloves; and Work exposes incumbent to mental pressures and/or emotional stress. Available as necessary after regular office hours for routine job duties or unusual incidents.

Licensing and Certification: Must possess and maintain licensure as a registered nurse in Iowa. Must possess and maintain a valid state of Iowa driver’s license or has the ability to arrange necessary and timely transportation for field travel.

Insurance Professional malpractice insurance is highly recommended. Auto insurance as required by Iowa law.

Criminal Background Check Satisfactory completion of criminal background check is required.

Starting salary will be commensurate with applicable work experience.

Pay: $23.32 - $37.31 per hour
