Care Manager, Adult Foster Care
Location: Lowell/Lawrence MA
REPORTS TO: Supervisor of Care Management Team
REQUIREMENTS/QUALIFICATIONS per AFC Manual:
Care Manager to provide psychosocial and counseling assistance for the AFC program.
Qualifications: The Care Manager must have
- Demonstrated leadership ability
- Bachelors’ degree, social worker license from the Massachusetts Board of Registration in Social Work, and at least two years’ recent experience working with elderly or disabled adults; or
- Bachelor’s degree, and two years’ clinical experience in the care of elderly or disabled persons
- Excellent verbal and written communication skills
- Commitment to quality care, and the multidisciplinary team approach
- Competence in CPR and First Aid
- CORI, Physical in the past 12 months, and Annual TB Test
- Reliable Transportation and internet
- Ability to drive or be transported to multiple locations within Massachusetts to meet required visit, intake or contract appointments

Provides social work assessment and/or referral as indicated to all appropriate AFC clients/families. Serves as liaison between client/family and community-based support services. Provides primary Social Work role to the Adult Foster Care members and caregivers. Starts in Intake process. Responsible for completion of psycho-social assessment and ensure that the Caregiver and Member meet the AFC requirements to enroll in program prior to scheduling the MDS with the Registered Nurse. Continues in all tasks required of a Care Manager under AFC regulations.
PERFORMANCE CRITERIA:
- Individual, group and family counseling.
- Initial and ongoing assessment of AFC qualified setting. Including completion of Environmental Inspection tool
- Selecting, training, evaluating, and supervising AFC caregivers in conjunction with RN.
- Informing members of and referring them to available community services
- Participating in development, implementation, and review of members’ care plans.
- Completing care management progress notes, reviewing AFC caregiver logs
- Reviewing and documenting the fire and safety procedures
- Participating in the discharge planning and implementation
