Job Title: Care Manager – Adult Foster Care Program Per Diem

About Us:
We Care 365 is a dedicated organization committed to delivering quality care and support for elderly and disabled adults through our Adult Foster Care (AFC) program. We are looking for compassionate, motivated, and organized individuals to join our team as Care Managers.
Job Summary:
The Care Manager will play a vital role in our AFC program, providing psychosocial support, counseling, and care coordination to ensure the highest quality of life for our members and their caregivers. This position will primarily serve members and caregivers in the Dalton/Pittsfield, MA area, with occasional statewide travel as needed.
- Starting Salary: This is a Per Diem role and employee will be compensated monetarily by the amount of visits they perform.
Key Responsibilities:
- Conduct psychosocial assessments and coordinate community-based support services.
- Develop, implement, and review individualized care plans in collaboration with caregivers and members.
- Provide training, supervision, and evaluation of AFC caregivers alongside the nursing team.
- Ensure compliance with AFC regulations, including monthly home visits, safety checks, and caregiver documentation.
- Maintain detailed case notes, assessments, and scheduling records in electronic databases.
- Facilitate caregiver and member education on health, safety, and program requirements.
- Engage with members and caregivers to build trust and foster independence.
- Serve as a liaison to external community resources and healthcare providers.
- Participate in team meetings, training sessions, and staff development opportunities.
Qualifications:
- Education: Bachelor’s degree required.
- Experience: Minimum 2 years working with elderly or disabled adults, with clinical experience preferred.
- Strong interpersonal, verbal, and written communication skills.
- Demonstrated leadership and ability to work in a multidisciplinary team.
- Current certifications in CPR and First Aid.
- Valid driver’s license and reliable transportation.
- Bi/Multilingual a plus!
Additional Requirements:
- Clear CORI background check
- Annual TB test and biannual caregiver TB screenings.
- Ability to travel primarily within the Dalton/Pittsfield, MA area, with occasional trips across Massachusetts.
- Proficiency in Microsoft Outlook, database management, and documentation systems.
Working Conditions:
- Frequent travel to client homes, primarily in the Dalton/Pittsfield area.
- Exposure to clinical settings, infectious illnesses, and household environments.
- Flexible work hours to accommodate client and caregiver schedules.
