Job Summary:
The Registered Nurse is responsible for delivering professional nursing care to our members, supervising caregivers, and ensuring compliance with all AFC regulations. This role involves frequent travel for in-home visits, documentation, and collaboration with the multidisciplinary team to maintain high-quality care standards.
Key Responsibilities:Clinical Oversight:
- Provide and supervise basic and skilled nursing services for members.
- Develop, review, and update care plans, ensuring alignment with member needs and AFC regulations.
- Conduct on-site visits for admissions, significant changes, monthly, semi-annual, and annual assessments.
- Ensure all nursing notes address changes in member conditions and are submitted promptly.
Caregiver Support:
- Train, evaluate, and supervise caregivers during visits.
- Conduct caregiver orientations before the commencement of personal care services.
- Monitor caregiver compliance with AFC requirements and address infractions as needed.
Documentation and Reporting:
- Prepare and complete Minimum Data Set (MDS) documentation, including pre-visit preparation, in-home review, and post-visit submission.
- Draft comprehensive care plans, narratives, and progress notes for submission to the member’s file.
- Ensure timely completion of semi-annual and annual health status reports and communicate findings with primary care physicians.
Compliance and Member Management:
- Manage a caseload of 40–60 members for full-time roles or 25–30 members for part-time roles.
- Ensure all records are complete and compliant with AFC program standards.
- Regularly review case files for documentation accuracy and completeness.
Team Collaboration:
- Work closely with care managers and other staff to schedule level-based visits and manage member needs.
- Attend monthly staff meetings and training sessions at designated office locations.
Qualifications:
- Current Licensure by the Massachusetts Board of Registration in Nursing.
- Two years’ experience in direct care for elderly or disabled adults.
- Competence in CPR and First Aid.
- Strong organizational, communication, and leadership skills.
- Reliable transportation for statewide travel.
- CORI background check, physical exam within the past 12 months, and an annual TB test are required.

Working Conditions:
- Frequent travel in Massachusetts, with up to 95% of time spent commuting.
- Work in home-care environments with potential exposure to clinical equipment, body fluids, and cleaning materials.
- Use of office equipment such as computers and fax machines for documentation and reporting.
