Key Responsibilities
- Greeting residents upon arrival and providing a warm welcome.
- Room readiness for any new admissions, and readmissions. This can include but is not limited to :
o Coordinate with housekeeping and maintenance staff to address any room-related issues.
o Conduct room inspections to ensure cleanliness and appropriate equipment is in place.
o Place welcome bags in the room for every new admission.
o Place welcome sign on the door for every new admission.
The completion of Admission Agreements with the resident and / or responsible party.Relay important information, and updates between departments to ensure seamless coordination.Email notifications for any new admissions and readmissions to the IDT / UR teams with outlined detailed admission information including appropriate medical information.Maintain an accurate and up-to-date room roster.Inputting new admissions to the facilities EMR system.Collecting and uploading appropriate documents to EMR.Run Coverage Eligibility verification and upload into EMR.Ensure all HCP / POA documentation is assigned to appropriate contact and uploaded into EMR. (If available)Update contact information after receiving signed Admission Agreement.Passes medications and gives treatments according to physicians orders and in accordance with facility policyProvides heedful care tasks, personal hygiene .services, housekeeping tasks and other related support services essential to the resident's healthFollows schedule in accordance to Pennsylvania Department of Human Services (DHS) standardsAlerts Administrator of any change in resident's physical or mental health statusReports and documents all resident incidents and emergency medical calls to administrative staffEnsures that residents receive meals during scheduled timesDoes not leave floor unattended without notifying a fellow employeeGives shift report on status of residentsDocuments in the resident recordsResponsible for notifying PC Manager or Charge Nurse of medications needing to be refilledAssures personal and staff compliance to residents' rightsProvides a clean and safe environment for residents, visitors, and other staff membersMonitors personal and staff compliance to all State and federal requirements including but not limited to blood borne pathogens, hazardous materials, infection control, and : fire safetySkills, Knowledge and Expertise
High School Diploma or equivalent1 Year long term care experience1 year med tech experienceBenefits
