Summary

As a Direct Care Worker at Weems Community Mental Health Center, you will play a vital role in supporting individuals with mental health challenges. This position is essential to our commitment to providing high-quality care and fostering a safe, nurturing environment for our clients.

Responsibilities

• Provide direct support and assistance to clients in daily living activities.
• Transport clients to and from group.
• Implement individualized care plans to meet the unique needs of each client.
• Monitor client behavior and report any changes or concerns to the appropriate staff.
• Facilitate group activities that promote social interaction and skill development.
• Maintain accurate documentation of client progress and incidents as required by HIPAA regulations.
• Educate clients about their rights and responsibilities within the care setting.

Qualifications

• Experience in caregiving or direct support roles, preferably in mental health.
• Ability to manage challenging behaviors with compassion and professionalism.
• Excellent communication skills for effective interaction with clients and team members.
• Demonstrated conflict management skills to resolve issues effectively.
• Ability to work collaboratively within a team environment.
