Welcome All Housing Services ( WAHS ) is seeking an experienced and driven Housing Stabilization Services Coordinator to lead our housing services program in and around the metro Twin Cities. WAHS is a Person-Centered social services agency. One of the appealing aspects of this position is that you will be helping community members who are homeless or in need of more affordable housing find suitable housing. It's a highly rewarding role. The Housing Stabilization Services (HSS) Coordinator will assist individuals diagnosed with mental, physical, and developmental disabilities, as well as those with substance abuse disorders or criminal backgrounds, in finding and maintaining housing within the community
Primary Responsibilities:
- Complete intake for HSS clients.
- Develop Housing Focused Person-Centered Plans (housing plans).
- Assist clients in finding and sustaining housing in the community.
- Assist clients with housing applications and completion processes.
- Drive clients to/from housing tours, appointments, etc.
- Accompany clients on tours of potential housing sites.
- Conduct regular meetings with clients to discuss housing options, progress, and troubleshoot any issues or setbacks.
- Assist clients in applying for housing support grants and funds.
- Support clients in applying for public programs aimed at housing cost assistance.
- Connect clients with community resources.
- Conduct outreach to the community and relevant agencies regarding housing issues.
- Maintain regular communication with waiver case managers, care coordinators, and mental health case managers to coordinate housing search efforts and resources.
- Collaborate with financial workers, landlords, and other financial aid agencies to gather resources necessary for sustaining housing in the community.
- Assist clients in managing and coordinating with Metro HRA (Housing and Redevelopment Authority).
- Attend team meetings and one-on-one meetings as required.
- Assist clients in planning for their move after securing housing.
- Assist with packing and coordinating moves.
- Refer clients (Waiver Clients) for Transitional Services.
- Follow up with clients regularly after their move to ensure housing stability.
- Assist clients in maintaining their housing stability.
- Utilize Crisis Management skills to prevent clients from losing stable housing.
- Maintain daily progress notes as required.
- Establish housing search and resource networks with counties and housing authorities.
- Research new affordable housing options for clients.
Position Requirements:
- Housing Stabilization Services Certificate (Online training available).
- Knowledge of affordable housing and housing resources.
- Comprehensive understanding of housing challenges faced by individuals with disabilities.
- Dedication to person-centered principles.
- Effective communication skills and commitment to supporting clients' decisions.
- Ability to collaborate with interdisciplinary teams to help clients locate affordable housing.
- Broad understanding of Disability Services.
- Minimum one year of experience working with Housing Access Coordination Programs.
- Experience working with waivered case managers and public housing programs.
- Knowledge of low-income and subsidized housing programs.
- Familiarity with navigating county programs.
- Completion of mandated reporter training (Available online).
- Ability to work with individuals with disabilities and chronic illnesses.
- Assist clients in identifying and implementing skills necessary for promoting independence.
- Able to pass a Minnesota Department of Human Services Background Study.
- Valid Driver’s License with fewer than 3 moving violations in the last 3 years, including DUIs.
- Maintain housing coordinator skills.
- Attend housing coordinator trainings.
- Possess a reliable vehicle.
- Maintain valid car insurance.
- Have access to a computer with internet and email.
- Maintain a cell phone.
Job Types: Full-time, Part-time, Contract
Pay: $20.00 - $23.00 per hour
