What are the responsibilities and job description for the Case Advocate position at Well-Care Kids First Child and Family?
Our Case Advocates are an integral part of our management team. Case Advocates provides assessments and other case management related duties to our clients. Case Advocates create, monitor and supervise treatment plans, develops and monitors clients assigned to their case loads. Maintains and organizes files, to include reviewing case notes; logging events and progress. Will conduct home visits, school visits and other related field duties to ensure compliance with program requirements. Some weekends and/or holidays may be required, as well as working on-call. Up to 25% travel, as well.

- Compassionate and able to relate to different clients with various needs.
- Strong verbal and written communication skills to explain to clients, family members and friends and professionals the case and care plan and maintain good case records.
- Ability to apply critical thinking and problem solving skills.
- Flexibility to change care plans if they are not getting the best results.
- Organization to manage several different cases at once
- Computer literacy to maintain and manage case records.
Requirements:
Bachelor’s degree in any social service field of study and atleast two (2) years of experience.
Job Types: Part-time, Contract
Pay: $9.00 - $12.00 per hour
