PURPOSE:
To coordinate and provide health care services within the adult day health/day care program.

Member Management
- Complete pre-admission Health Assessment for enrollment into the program
- Implement and document the nursing component of the individual care plan, including orders or suggestions, if any, of involved health care professionals.
- Monitor member’s response to medical treatment plan and nursing interventions and revise care plan if needed.
- Coordinate with other staff and facilitate members use of community health services as needed, i.e. specialized services such as speech, physical therapy, occupational therapy, home health, in-home or overnight respite, referral to neurologist, etc.
- Communicate with family caregivers and staff about health concerns, changes and conditions of members.
- Report and document results of nursing assessment, care provided and member’s response to the care.
- Communicate with members’ physicians as needed to update medication orders, report vital signs, physical and emotional problems, etc.
- Coordinate with staff to implement regular health-maintenance services (i.e., vital signs, weight, dental health, general nutrition and hygiene) of members periodically, documenting in member records and notifying caregivers of any significant changes.
- Supervise administration of medications, first aid and emergency care and document appropriately.
- Supervise the provision of modified and therapeutic diets or supplemental feedings.
- Provide other direct nursing services; regularly report/observe functional changes of members.
- Conduct monthly member health education programs as required by agency policy.
Family Support and Education
- Participate in scheduled family conferences as team member to provide information about health concerns and conditions of members; counsel with caregivers, taking any follow-up action appropriate.
- Communicate with family members by telephone and in person regarding day-to-day problems and concerns.
- Provide family members with written information needed, i.e. medications, behavior management, disease-specific (Alzheimer’s Disease or stroke, etc.), nursing homes, etc.
Staff/Program Development
- Conduct with staff regular drills in handling different kinds of emergencies and document as outlined in the agency policies.
- Assist with the supervision of daily activities in the absence of the Program Director.
- Maintain medical supplies as required in state standards.
- Assist with Guilford County Health Department Nurse Monitor of Center health care services for state certification requirements.
- Assist with the implementation of USDA requirements for nutrition.
- Network with other health-care professionals to promote center services and maximize provision of these services..
Qualifications:
- Shall be at least 21 years of age and have a current license to practice nursing in North Carolina, with RN / LPN status.
- Shall have current CPR and First Aid Certification.
- Shall have at least two years work experience, preferably in the human services and/or gerontological nursing area.
Possess the following characteristics:
- Maturity, good judgment, emotional stability, ability to make decisions and set goals.
- Managerial and administrative skills including the ability to supervise staff and to plan and coordinate meaningful staff training.
- Knowledge and understanding of the physical and emotional aspects of aging, the diseases and infirmities associated with aging and related medications and rehabilitative measures.
