Overview
The ideal candidate will have aWe are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. This is supplemental, PRN position is ideal for working professionals seeking additional income. In this role, you will provide medication administration training to our direct care employees and oversee medication management for our residents. Our facility operates in a group home setting, not a hospital environment. All of our residents are independent and do not require extensive care. This is a flexible position, requiring approximately two hours of in-person visit per week, with the remainder of the work completed remotely. strong background in outpatient care and experience with diverse patient populations, including adults with mental health and Physical disabilities. As a Nurse Practitioner, you will play a vital role in providing high-quality medical care, conducting assessments, and developing treatment plans to enhance patient health outcomes. Your expertise in psychopharmacology and infectious disease care will be invaluable in delivering comprehensive healthcare services.
Salary is negotiable and based on experience, flexibility, and work load.
Duties
- Conduct thorough patient assessments and evaluations to determine health needs.
- Develop and implement individualized treatment plans based on patient diagnoses.
- Provide psychopharmacological management for patients requiring medication therapy.
- Collaborate with physicians and other healthcare professionals to ensure coordinated care.
- Manage outpatient services, including follow-up visits and ongoing patient education.
- Perform procedures as needed, including DME (Durable Medical Equipment) assessments.
- Provide care for patients with infectious diseases, ensuring appropriate protocols are followed.
- Document patient encounters accurately in electronic health records (EHR).
- Participate in clinic operations, ensuring compliance with healthcare regulations and standards.
Requirements
- Valid Nurse Practitioner license in the state of Minnesota.
- Experience in Group Home settings is preferred; experience.
- Strong knowledge of psychopharmacology and ability to manage medication therapies.
- Familiarity with DME processes and procedures.
- Ability to work collaboratively within a multidisciplinary team environment.
- Excellent communication skills, both verbal and written.
- Commitment to providing compassionate, patient-centered care.
- Ability to work effectively in fast-paced clinical settings, including ER or OR environments if needed.
