Job Summary
WellCare Support Inc. is hiring Registered Nurses (RNs) to conduct patient assessments and create care plans for ADLs. Starting at $50 per assessment, this role requires travel statewide to patient locations across Arkansas. Join our team and make a difference in home healthcare!
Responsibilities
- Conduct comprehensive patient assessments to determine eligibility for home healthcare services.
- Develop and document individualized care plans based on assessment findings.
- Communicate care plans to caregivers, families, and the agency to ensure proper follow-up.
- Monitor patient progress and update care plans as needed, ensuring compliance with regulations.
- Maintain accurate and timely documentation of assessments and patient interactions using electronic health records.
- Educate patients and families on health management, medication adherence, and treatment plans.
- Ensure compliance with home healthcare regulations, policies, and best practices.
- Promote patient safety through adherence to clinical guidelines and participation in audits.
- Serve as a liaison between caregivers, families, and the agency to ensure coordinated patient care.
Qualifications
- Active Registered Nurse (RN) license
- Minimum of 2 years of nursing experience, preferably in home healthcare, acute care, or a related field.
- Strong assessment, critical thinking, and problem-solving skills.
- Proficiency in medication administration, vital signs monitoring, and patient assessments.
- Familiarity with electronic health records (EHR) systems; experience with Epic is a plus.
- Ability to effectively educate patients and families on treatment plans, medication management, and health conditions.
- Strong verbal and written communication skills to collaborate with caregivers, families, and healthcare professionals.
- Ability to work independently while managing multiple patients in a home health environment.
- Valid driver’s license and reliable transportation to travel to patient locations statewide.
