Wellhaven Home Care Columbia is looking for a driven Nurse Supervisor to:
- Ensure compliance with South Carolina Department of Health and Human Services policy regarding providers on the Community Long Term Care and the Department of Disabilities and Special Needs waiver programs
- Conduct initial and periodic supervisory nursing visits to our clients under the DHHS waiver programs
- Ensure the highest quality of caregiver service delivery in conjunction with our Director of Operations
- Cultivate healthy relationships with clients/caregivers
- Provide caregiving services as needed
- Support the overall vision and mission of Wellhaven Home Care

Responsibilities and Duties
Ensure compliance with the South Carolina Department of Health and Human Services policy
- Maintain an understanding of SC DHHS policies and platforms regarding the CLTC and DDSN waiver programs
- Record necessary documentation in WellSky Personal Care and Phoenix Provider Portal
- Help define the client's Plan of Care with the Director of Operations according to the waiver authorizations
- Communicate in a timely manner with Wellhaven Home Care staff and DHHS Case Managers regarding client status
Conduct initial and periodic supervisory nursing visits to our clients under the DHHS waiver programs
- Coordinate visits with clients and associated support structures (family members, case managers, etc.)
- Provide a detailed assessment of the client’s well-being, according to DHHS guidelines
Ensure the highest quality of caregiver service delivery in conjunction Director of Operations
- Assess the quality of services provided by the caregiver during supervisory visits
- Provide feedback to the caregivers regrind specific areas of care in need of focus
- Assist in the ongoing training of the caregiving staff
Provide regular caregiving duties as needed
- Serve as an on-call caregiver, when possible, to fill- in scheduling gaps or begin care with a new client.
Support the overall vision and mission of Wellhaven Home Care
- Maintain active engagement with our Irmo office
- Serve as a point of contact for Wellhaven Home Care matters, managing communications through phone, mail, email, text and other mediums
- Maintain accurate data on company platforms
- Faithfully and professionally represent the Wellhaven Home Care brand
Qualifications and Skills
Requirements and Qualifications:
- Valid Registered Nurse license in the state of South Carolina
- Experience with SC DHHS waiver programs
- Excellent written and verbal communication skills
- High proficiency with computers, software programs, online platforms, and mobile devices
- Must be able to pass Drug Screen and Background Check
Ideal Candidate will possess:
- Nurse management or leadership experience
- Experience in the Senior, Home Health, or Home Care industries
- Experience with computer office suite applications (Apple, Google, Microsoft)
- Experience with data entry platforms
Job Type: Part-time
Pay: $30.00 - $45.00 per hour
Expected hours: 6 – 10 per week
