The overall responsibility of the Unit Coordinator at Wellington Place at Biron is to oversee the day-to-day operations including but not limited to: care planning, change of status, med changes, anything resident related, training of new staff, assist with staff performance review, Inspection Binder for fire drills, elopement drills, emergency disaster drills, and working the floor as needed due to shortage of staff alternating shifts and on-call.

Qualifications:

Knowledge and Abilities:
- Comprehensive knowledge of the Wisconsin Illinois Senior Housing dba Wellington Place at Biron Policies and Procedures.
- Basic knowledge of administration, supervision, management principles and practices including organization, procedural techniques, decision making, and management controls.
- Basic knowledge of dysfunction, dementia, Alzheimer, and social problems.
- Ability to plan, organizes, assign, supervise, and evaluate the work of others.
- Ability to establish and maintain effective working relationships.
- Ability to communicate effectively, orally and in writing.
- Ability to serve as a leader and as a positive role model for internal and external customers.
- Knowledge in computers, which includes all Microsoft products
- Training and Experience:
- Must have at least 3-5 years working as a C.N.A. / R.A., with management experience or oversight of other co-workers
- Have been trained in fire safety, first aid, Block I, and or C.N.A. certified and the basic skills set needed in order to perform the job of a Unit Coordinator.
Essential Duties and Responsibilities include but not limited to the following:
- Attend Stand up meetings every morning at 10:00am
- Will go over all resident issues
- Staff related issues
- What was done the previous day
- What needs to be done today
- To work alongside Care Coordinator, and Administrator with the operations of the facility and to assist as needed.
- Answering the telephones in a timely and professional manner and directing calls to correct supervisor.
- Do walking rounds at the beginning, during and before you leave.
- All duties as described in CNA/RA job description regarding all resident care needs.
- Report to work for any of the following
- Emergency issues
- Shortage of staff
- Work any shift where needed
- No-call, No-shows
- To assist Care Coordinator on investigations if needed
- Giving direction to staff so that they are completing their job duties and avoid idle time and offering insight of more efficient ways to perform job duties.
- Follow-up with Administrator and/or Care Coordinator on any issues that arise daily.
- Maintains a safe and secure working/living environment by monitoring staff, building maintenance and housekeeping.
- Assist Care Coordinator with employee evaluation on a 90 day and annual basis.
- Addresses and resolves staffing issues as needed. Record of Conversations (ROC) are to be used when dealing with issues that arise with staff which includes staff not performing their job duties, staff being unprofessional with co-workers etc. If you are hearing complaints from one staff who heard it from another it is hearsay.
- Enforce safety issues; gait belts, Pal-Lift, and Hoyer. Ensure that staff fills out incident reports, assist with incidents reports as needed.
- Administrator and or Care Coordinator to be called for all injuries to staff and residents immediately.
- Assess and follow up on Resident or Families complaints by informing Administrator or Care Coordinator immediately.
- Have ISPs completed the month prior to when they are due and send out an Invitation letter to set up a Care Conference and follow up to ensure that it is returned. When doing the ISP, a Functional Screen needs to be done to see if there is a level of care change needed.
- Work with Care Coordinator on all admissions
- Make changes to ISP when needed and send out a change letter
- Follow up with a letter if there is a level of care change
- Serve as an employee relations liaison by working closely with the Administrator, and Care Coordinator to recruit and retain employees.
- Document on all medication changes or updates after MD appointments
- Order Medications as needed, manage cycle fill
- Transcribe all new or changes to medications timely
- Arrange for transportation for resident for all medical appointments
- Have your Continuing education done either before or on its due date
- Pendants/Wander guards are working
- Keep the inspection binder up to date
- Have a state ready binder together
- Keep roster for both staff and residents (update as needed
- Copy of all fire drills
- Copies of all inspections
On-Call Responsibilities: On-Call rotating weeks with Care Coordinator
Training and Experience:
Must have at least 3-5 years working as a C.N.A. / R.A., with management experience or oversight of other co-workers
Have been trained in fire safety, first aid, Block I, and or C.N.A. certified and the basic skills set needed in order to perform the job of a Unit Coordinator.
