POSITION TITLE: Clinician Case Manager
PROGRAM: Functional Family Therapy
General Statement Of Responsibilities
The Clinician Case Manager is responsible for direct services with adolescents and their families including but not limited to evaluation/assessment; home & community based services; outreach; case and care management; and family therapies; record keeping; collaboration with other professionals and team members and related community services (including consultation and/or community and/or school placements).
SUPERVISION: Supervised by: Functional Family Therapy Supervisor
Supervises: Interns and/or those designated by supervisor.

QUALIFICATIONS:
Must have the ability to maintain the utmost confidentiality of both agency and client matters. Must understand and be able to display the clinical knowledge and expertise necessary to perform crisis intervention. Must be able to display knowledge and expertise in clinical theory and practice as it relates to the populations served by Wellmore. Must be capable of working as a part of a team and be able to effectively communicate with both families and professionals (in writing and verbally). Working knowledge of the greater Waterbury area is a must! Bi-lingual (Spanish-English) may be required.
Education & Experience
A Master’s Degree in marriage and family therapy, social work or related human services field with at least a one-year clinical internship working with children and/or adolescents or families is required. A Connecticut license as a Marriage and Family Therapist, Professional Counselor, Licensed Clinical Social Worker or Doctorate Psychologist preferred. MSW candidates must possess a LMSW at a minimum. Two years post graduate clinical experience working with children, adolescents or families is preferred. Experience in family therapies as well as family systems theory and practice is preferred.
Licensure/Certification/Registration
Valid driver’s license, reliable transportation and valid auto insurance; CPR and first-aid.

Examples Of Responsibilities
- To provide comprehensive evaluations, treatment plans and treatment of adolescents and their families utilizing Functional Family Therapy. This includes evaluation/assessment; home and community based services; outreach; case and care management and family therapies.
- To implement treatment plans that may include psychotherapy and clinically required support services including but not limited to translation, transportation ( only as per agency policy ) advocacy and linkage to other needed services.
- To provide community based outreach and treatment services as defined by the treatment plan and/or client need, in a variety of settings including but not limited to: agency offices sites, client homes, schools, shelters, police stations, etc.
- Maintain contact and provide follow-up with families requiring this service . This will include daily home visits and home based therapies.
- Maintain confidential and/or any other required program documents.
- Maintain a level of productivity as determined by the Executive Director and assigned by the program supervisor.
- To work collaboratively as a member of a team with other professionals and concerned parties involved in cases.
- To assist in the organization, marketing, planning and implementation of the program.
- Actively participate in FFT and internal trainings and staff meetings.
- To serve on agency and/or community committees.
- Actively participate in supervision and report directly to the supervisor.
- To supervise assigned staff and/or student interns.
- Other such duties as assigned by the Supervisor.
