POSITION TITLE: Case Manager
PROGRAM: Residential Programs
GENERAL STATEMENT OF RESPONSIBILITIES:
Residential Case Managers are responsible for direct and indirect substance abuse treatment services to behavioral health clients seeking or obtaining services from the agency. Case Managers are usually assigned a caseload that consists of 60% direct service time and 40% indirect service time. Direct service includes assessment interviews, individual, family and group counseling. Indirect service time includes supervision, staff meetings, telephone calls, treatment authorizations and all paperwork associated with client care.
SUPERVISION: All duties are performed under the supervision of the Program Manager and/or Vice President of Adult Services.
EXAMPLES OF RESPONSIBILITIES:
- Develops positive working relationships with supervisor, co-workers and members of the clinical team.
- Develops appropriate therapeutic relationships with clients.
- Abides by the Code of Ethics
- Conduct and write assessments within the contractual, funding and licensing timeliness specified in the Program.
- Collaborate with clients in the development of treatment plans, treatment plan reviews, and discharge/continuing care plans within the contractual, funding and licensing timelines.
- Facilitate psycho-educational and therapy groups, provide individual counseling.
- Include members of the client’s support network in the treatment process.
- Develop effective networking relationships with outside contractors, referral sources and community agencies.
- Supervise urine samples for testing, perform breathalyzer testing and collect samples for hair testing.
- Provide coverage for co-workers during vacation and personal leave time.
- Conduct room checks/searches in residential programs.
- Organize and present clinical information to managed care companies for the purposes of treatment authorizations and continued stay reviews.
- Other job-related duties as assigned.
Qualifications
QUALIFICATIONS:
- Employee must demonstrate an ability to develop within the position and demonstrate competency in 12-Core functions of an addiction counselor, including assessment, treatment planning, counseling and record keeping.
- Individual is required to have DARC/associate's degree and/or CADC credentials.
Salary : $20 - $23
