POSITION TITLE: Per Diem Crisis Intervention Clinician
PROGRAM: Urgent Crisis Center
The Urgent Crisis Center (UCC) is a behavioral health urgent care center that provides crisis evaluation, de-escalation, and community coordination of care for children and youth experiencing a behavioral or psychiatric crisis. All evaluations, assessments, safety planning, and initial recommendations for follow-up care must occur within a 24-hour time period. All staff are required to work their assigned schedule but may be asked to shift/alter this in response to high workflow time periods. All Clinicians work a minimum of 2 agency observed holidays each calendar year.

EXAMPLES OF RESPONSIBILITIES:
- To provide comprehensive crisis evaluations, treatment plans and treatment of children, adolescents and families who present to the Urgent Crisis Center.
- To assess and implement crisis stabilization plans that include immediate tasks for crisis management and connection to clinically indicated resources and services.
- To provide community-based outreach and treatment services as defined by the treatment plan and/or client need, in a variety of settings including but not limited to: agency offices sites, client homes, schools, emergency rooms, shelters, police stations, etc.
- To complete clinical documentation thoroughly within required timelines to ensure appropriate and ethical care. Per diem clinicians are expected to complete documentation, such as diagnostic evaluations and case management notes, before leaving their shift for the day.
- To respond to any feedback or revisions in a timely fashion to ensure ongoing compliance with agency and regulatory standards.
- Maintain contact and provide follow-up with families as needed, including phone and collaborative support.
- Maintain confidential and/or any other required program documents, including but not limited to insurance and/or managed care required documentation.
- Maintain a level of productivity as determined by the program supervisor.
- To assist program management or agency leadership with assigned tasks or projects that develop to support the communities we serve.
- To work collaboratively as a member of a team with other professionals and concerned parties involved in cases.
- Other duties as assigned by the Urgent Crisis Medical Director.
Qualifications
Qualifications:
Must have the ability to maintain the utmost confidentiality of both agency and client matters. Must understand and be able to display the clinical knowledge and expertise necessary to perform crisis intervention. Must be able to display knowledge and expertise as it relates to the populations served by Wellmore, including bilingual and bicultural needs. Must be capable of working as a part of a team and be able to effectively communicate with both families and professionals (in writing and verbally). Must have comfort with navigating and documenting within an electronic health records and use communication platforms, such as Microsoft Teams and Microsoft Outlook. Must be willing to work a flexible schedule that meets the needs of the UCC program. Must be able to display an understanding of the diverse cultural communities prominent in greater Waterbury, Danbury and Torrington communities. Working knowledge of the greater Waterbury/Danbury/Torrington areas is preferred
Bi-lingual (Spanish-English) preferred.
EDUCATION & EXPERIENCE:
A Master’s Degree in marriage and family therapy, social work or related human services field with at least a one-year clinical internship working with children and/or adolescents or families is required. Two years’ post graduate clinical experience working with children, adolescents or families is preferred. Experience in family therapies as well as family systems theory and practice is preferred.
LICENSURE/CERTIFICATION/REGISTRATION
Must have a CT license as a mental health provider at the associate level (ie: LMSW, LPCA, LMFTA); full CT licensure is preferred.
Valid driver’s license with good driving record, reliable transportation and valid auto insurance.
Salary : $36 - $44
