Job Description
To provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual OB/GYN patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote through the integration and functions of case management, utilization review and management.
- Providing mental health counseling to individuals
- Conducting initial assessments of clients' situation to determine needs and goals
- Researching and advocating for appropriate public assistance resources for clients
- Communicating with clients' care teams
- Referring individuals to appropriate counselors within the Wellness Pointe system
- Ensuring that all case files, and other records strictly comply with policies, regulations and procedures
- Actively participating in ongoing training as needed in order to meet all certification standards and credentialing policies
- Carries out other duties as assigned.
Qualifications
- Master’s Degree in Social Work.
- Current Texas License
- Knowledge of mental health assessment, treatment planning
- Bi-lingual English-Spanish speaking proficiency preferred, but not required.
- Eligible to be a Medicaid and/or Medicare provider.
- Mental and physical health sufficient to meet the demands of the position.
- Requires a valid Texas Driver's License, appropriate driving record and auto liability insurance that meets the minimum required by State of Texas statute.
- Considerable knowledge of available community resources and of their organization, purpose and use.
- Ability to maintain clinical records and prepare related reports.
- Ability to work cooperatively in an interdisciplinary team setting.
- Ability to express ideas effectively.
- Ability to exercise judgment and make independent decisions based on professional knowledge.
