Description

Housing Integrated Services (HSI) is a specialized program that provides comprehensive and intensive field-based community mental health services designed to meet the unique needs of youth, ages 18-25, in Permanent Supportive Housing (PSH) who are formerly homeless and have a Severe Mental Illness (SMI) or Serious Emotional Disturbance (SED). HSI services are provided as part of an integrated model in which the Los Angeles County (LAC or County) Department of Mental Health (Department or DMH), Department of Health Services (DHS), and Department of Public Health (DPH) partner to respectively provide mental health services, intensive case management services (ICMS), and substance use services, as appropriate, to Clients who are in or have been matched to permanent supported housing.

 

Wellnest serves Transition Age Youth (TAY) at our five story100%permanent supportive housing development. As a team member, therapists handle cases as assigned, performs interviews and collects information from clients, identifies symptoms, makes diagnoses and develops a therapeutic or treatment plan. Therapists conduct individual therapy sessions, run therapy groups for TAY & TAY families, plan and administer therapeutic treatment in a field-based setting at the Nest on Florence or elsewhere. Therapists consult with the clinical supervisor, medical doctor, Wellnest peers or other specialists and timely complete required progress notes and paperwork. Therapists must meet or exceeds productivity and timely expectations. Other duties may be assigned in this program that includes providing emergency services 24 hours a day, seven days per week.

 

 

ESSENTIAL FUNCTIONS:

  • Builds trust and rapport with individuals. Assess individuals and complete assessments, re-assessments, and client treatment plan that are required by the agency and funders.
  • Implements client treatment plans, emphasizing coordinated strategies for addressing mental health/substance abuse issues, relapse prevention, medication management, and any other risk factors that could impede permanent housing and provide ongoing assessment of client progress in attaining goals.
  • Provides complete documentation, meeting medical necessity, of all services and plans provided to clients.
  • Keeps highly organized files for each client and enter appropriate data within agency and funder compliance.
  • Coordinates services and collaborates with other service providers involved in the clients’ care.
  • Links clients to psychiatry services and other supportive services internally or externally.
  • Provides skill building, rehabilitation and therapy services to individuals and groups.
  • Utilizes a harm reduction model when working with clients with chemical dependency or abuse issues.
  • Utilizes evidence-based practices and best practice models, when appropriate, to meet each client’s individual needs.
  • Participates in case conferencing meetings and other community meetings.
  • Coordinates and collaborates with community, law enforcement, and other service providers and programs.
  • Participates in individual clinical supervision, group clinical supervision, and administrative supervision.
  • Represents the company at community services, funder events and events related to outreach for clients and supporters of the organization.
  • Participates in trainings and meetings as assigned.
  • Other duties as assigned.

 

 

REQUIRED QUALIFICATIONS

Minimum of a master’s degree or equivalent (MSW, MA, Ph.D., PsyD, or MS) plus clinical experience and/or training directly related to adolescent counseling. Supervision for clinical hours for master’s level clinicians obtaining hours toward licensure is provided. Current registration with licensing board is required. Bilingual Spanish speaking ability is preferred.

 

 

WORKING CONDITIONS

Employee regularly needs to work out in the field, in a variety of environments. Employee may be required to work flexible, evening and/or weekend hours for time-sensitive projects.

PHYSICAL DEMANDS

  • Ability to talk or hear in order to give and receive information and instructions
  • Ability to stand, walk, sit, use hands to finger, handle or feel objects, tools or controls
  • Ability to reach with hands and arms
  • Ability to use computer keyboard up to 50% of the day
  • Visual acuity, ability to adjust focus and peripheral vision for work on a laptop or other computer equipment
  • Lift and/or move up to 25 pounds
  • Computer literate (basic working knowledge of Microsoft WORD, Excel, and Outlook) and data/electronic records program currently in use
  • Able to use telephone, office copier, calculator, fax machine, computer printer and scanner, or other equipment as required

  • This position is also eligible for an annual $5,000 intensive services differential

  • Annualized Salary Range: $67,704 $83,410plus incentive compensation opportunities
