Job Purpose:
Our next Lead Care Coordinator – Comprehensive Community Services will be responsible for coaching, modeling and accompanying uncertified Care Coordinators as they acclimate to their role. This position will manage cases by providing community-based family support for mental health and behavioral needs of youth and young adults. Service provision includes assessment of needs, referral, connection to network and/or community services, and planning for family sustainability.
Qualifications:
- Bachelor’s Degree in Social Work, Psychology, Nursing, Occupational Therapy OR a bachelor’s degree with work experience in human services required; Master’s degree preferred.
- Minimum of one (1) year experience as a Care Coordinator in the Comprehensive Community Services (CCS) program required.
- Experience working with emotionally disturbed children and their families required.
- Ability to manage cases dealing with the courts and public school systems.
- Understands family systems and dynamics.
- Knowledge of community resources.
- Knowledge of conflict resolution and mediation practices.
- Knowledge of AODA issues.
- Knowledge of sexual abuse issues.
- Demonstrates ability to use a strength-based and community centered approach in working with the child/family.
- Communicates effectively through verbal, non-verbal and written communications. Effective communication involves the timely exchange of accurate and relevant information
- Works efficiently, organizes time effectively, maximizes use of available resources, successfully balances the competing demands of multiple projects by setting priorities and communication and meeting deadlines, asks for help when necessary
- Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
- Must be honest, dependable and able to meet deadlines; Self-motivated and able to work independently.
- Physical exam, drug screen, motor vehicle report, and background checks are required for this position
- A valid Wisconsin Driver’s License or occupational driver’s license, reliable transportation and insurance is required.
Duties:
Direct Services
•Maintains a caseload of 6-8 youth/families.
•Demonstrates individualized and creative planning for families that is aligned with the beliefs, values, and culture of the family.
•Facilitates the Clinical Therapist Team Process, including the engagement of informal and natural supports.
•Provides or secures support and crisis/emergency services for the child/family (and informs team within 24 hours).
•Consults others when appropriate and attends scheduled consultations and supervisions.
•Collaborates with other necessary individuals the youth and family may have contact with such as, case managers, attorneys, teachers, therapists, mentors, schools, etc.
•Utilizes Wellpoint Care conflict resolution procedures when appropriate.
•Participates in the implementation and planning of monthly family events, including reasonable efforts to ensure family attendance.
Documentation and Information Sharing
• Completes all necessary paperwork timely and in a strength-based and quality manner per Wellpoint Care requirements, including change of placement forms, SARs, referrals, POCs, progress notes, evaluation instruments, consent forms, etc.
•Maintains regular contact with providers.
Training and Staff Assistance
•Instructs new staff on Wellpoint Care policies and procedures and maintains follow through on policies with existing staff.
•Provides coaching for staff by attending Clinical Therapy Team Meetings/POCs.
•Reviews documentation for accuracy and strength-based language, including Progress Notes, Plans of Care (utilizing the POC Rubric), Out-of-Home Care Authorizations, Transitional/Disenrollment Plans, and Community Safety Plans for families, as needed.
Consultation
•Assists supervisor with providing clinical direction, consultation, crisis intervention, and conflict resolution for care coordinators and Clinical Therapy Team members that support family centered and strength-based service delivery.
Quality Assurance/Quality Improvement
•Utilizes reports and data provided by Wellpoint Care to continuously improve the care provided to youth and families, as needed.
•Assists supervisor in reviewing and ensuring compliance with standards for all written documentation.
Team Leadership
•Leads assigned department personnel, which may include assisting with oversight and providing input into hiring, learning, scheduling, evaluating, as well as ensuring accuracy, timeliness, and the completion of all work performed by team members.
•Assist team members by meeting as needed to resolve problems, manage projects, track goals, and review work processes and procedures.
•Represents team and/or department at meetings, events, and learning as needed.
•Assists supervisor with additional leadership and administrative responsibilities as requested.
