POSITION SUMMARY : Works with internal clinical staff to assist the people we serve with their administrative needs. Serves as a resource broker and coordinator of services and documentation for outpatient services. This position allows a hybrid model of working some days from the office and some days from home.
Pay Range & Benefits :
CMI - $28.96 / hr - 60,237.00 annually (Bachelors Degree)
CMII - $30.05 / hr. - $62,504 annually (Master's Degree)
Language Differential - $1hr - $2.50 / hr for bilingual proficiency in Spanish & ASL
ESSENTIAL JOB FUNCTIONS :
- Support medical providers by meeting with people we serve either in person or by phone to complete any documents that are needed. Documents may include Consent to Treat, Consent for Telehealth, Authorizations to share personal health information for emergency contacts, PCP's, outside providers and any others as needed.
- Complete regular documents for electronic medical record such as CCARS for the state and treatment plans as necessary.
- Act as a contact person for people on the Psychiatry Only level of care and assist providers in coordinating letters or forms needed by the people we serve.
- Help people we serve in obtaining basic needs by referring them to resources for transportation, housing, benefits, etc. and linking them with primary care, dental care, psychosocial rehab, employment services, or other services they may need.
- Complete transfer paperwork for people transferring into or off the Psychiatry Only level of care.
- Outreach people assigned to you whose medications have expired and encourage them to schedule with their provider.
- Discharge people from the agency who have moved or disengaged within 45 days of their medications expiring.
- Complete readmission paperwork for people desiring to be reopened within 60 days of being discharged from care or direct them back to Access after that.
- Offer support, assistance, consultation, and education to families, friends, landlords, employers, community agencies, and others who encounter the people we serve in order to maximize benefits and minimize problems they may experience.
- Reach out to program eligible folks and inform them of and educate them about available services at WellPower and in the community.
- Maintain a trauma informed environment of wellbeing.
- Provide all services with an equity lens and an inclusive and welcoming attitude.
- Other duties as assigned.
EDUCATION : Bachelor (CM I) Degree in psychology, social work, or other related human services degree.
EXPERIENCE : One year experience in mental health or human services preferred.
