Pay Range: $33.17/hr - $43.12/hr

Position Summary
The LVN coordinates and administers medical services to patients, including assessing, screening, educating and treating patient conditions.
Required Qualifications
- Current license as Licensed Vocational Nurse with the State of California required.
- Two plus years of experience in a clinical environment with a diverse adult population OR an equivalent combination of education and work experience preferred.
- Bilingual in Spanish preferred.
- Current First Aid and CPR Certification.
- Knowledge of community resources including Medicare, Medi-Cal, Supplemental Security Income (SSI) and the county’s General Assistance Program.
- Knowledge of medications, usages, interactions and effects.
- Ability to observe, assess, and record symptoms, reactions, and progress.
- Ability to react calmly and effectively in emergency situations.
- Ability to educate patients and/or families and to provide instruction on proper care and treatment.
- Ability to maintain detailed records.
- Ability to establish and maintain positive, professional relationships.
- Proficient computer skills, including electronic health record, word processing and e-mail, preferred.
- Excellent verbal and written communication skills.
- Adherence to infection control procedures, including but not limited to standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking, and social distancing.
- Assist and support the Center/Department/Program to meet standards of High Reliability.
Essential Responsibilities
- Administer injections according to standing order.
- Perform LVN duties, including preparing rooms, rooming patients, taking medical histories,recording vital signs, taking temperatures, and performing venipuncture and immunizations.
- Follow standardized clinic policies and protocols for patient treatments and conditions such as tuberculin skin tests, nebulizer treatments, glucose monitoring, hemoglobin finger sticks.
- Encourage patient’s general wellbeing through health promotion.
- Refer patients to specialists.
- Keep accurate records for reporting to various funding and inspecting agencies.
- Maintain files for regulatory compliance and inspection.
- Ability to work occasional overtime to cover day, evening or night shift as needed.
- Maintain immunization files for the county registry.
- Train CNAs, interns and new hires in all duties.
- Oversee CNAs, MAs, Shift Monitors as needed.
- Inventory medications and supplies; rotate inventory on a regular basis to minimize expirations.
- Review patient medical charts.
- Work with patients experiencing homelessness and co-occurring disorders such as mental health, chronic medical conditions, and substance use disorders at a high acuity care facility requiring 24/7 patient support and monitoring.
- Respond quickly and effectively to crisis situations and potentially hostile situations.
- Perform other duties as assigned.

Physical Demands and Work Environment
The work environment is similar to a shelter for the unhoused. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment can range from quiet to moderately loud; incumbent must be able to focus in an environment with many distractions.
The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bio-hazardous materials (bodily fluids including blood and urine) and hazardous chemicals.
