General Summary

Supports health system improvements and implementation initiatives. Through data collected from patient calls, this position identifies, elevates, and communicates gaps and opportunities to improve patient care quality and safety. This position conducts outreach to specific populations in the community through PCORI-sponsored efforts.


Duties and Responsibilities

Essential Functions:

  • Conducts outreach to patients by telephone or other electronic means to engage patients in structured interviews to gather patient reported outcome data.
  • Compiles data for and prepares regularly scheduled or special reports, analyses, and statements.
  • Elevates and communicates barriers and opportunities with improvement recommendations through established channels to improve care quality and safety.
  • Applies process improvement principles and quality improvement tools and techniques to identified processes/systems/ tasks.
  • Maintains effective working relationship with peers of the Quality & P4V department across the System as well as working jointly with other departments on community related programs and events. Develops a network of internal resources who can lead wellness programs on various topics.
  • Provides general program planning support as needed.
  • Communicates findings and improvement strategies to project stakeholders through regular meetings and reports.
  • Provides exceptional customer service by addressing patients in a professional, positive, and caring manner.
  • Conducts outreach to specific populations in the community through PCORI-sponsored efforts, building relationships and promoting health initiatives.

Travel Requirements:

  • Estimated Amount: 20% - Travel within the local service area. Site visits include provider and staff engagement. Potential for community partner meetings.



Qualifications

Minimum Education:

  • Associates Degree Required
  • Bachelors Degree Preferred

Work Experience:

  • 2 years Relevant experience. Required

Driver's License Statement:

Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
