As a Bereavement Coordinator with the Frankenmuth Hospice team at Wellspring, you’ll be part of a team of nurses, CNAs and senior support staff committed to helping seniors and families within the Saginaw, Frankenmuth, and surrounding areas. In this position, you'll bring a Bachelor's Degree in Social Work or a human services related field, with two years of work experience in a healthcare setting. You'll also bring experience with the Medicare Hospice insurance program and participation in community activities (preferred).

As a Bereavement Coordinator, you will:
- Implement the bereavement component of the team plan of care following the initial bereavement contact; this may include, but is not limited to, monthly bereavement mailings, support groups, telephone contacts or scheduled visits and coordination of the hospice memorial services
- Attend the interdisciplinary group meeting and participate in evaluating all services delivered to the patient and family
- Maintain the clinical record on families/caregivers receiving bereavement services Participate in volunteer training as designated
- Compile a monthly or quarterly bereavement activity report and review with Patient Care Coordinator and/or Quality Coordinator; the report includes, but is not limited to:
- number of patients/families on active bereavement care
- number of discharges
- number of personal patient/family contacts
- number of telephone patient/family contacts
- results of Bereavement Satisfaction Surveys
- Coordinate mailing, compilation and reporting results of a Bereavement Care Satisfaction Survey sent at the time of discharge of a family from bereavement care
- Acts as Camp Director for Camp Hope according to the camp guidelines
