As the Hospice Clinical Manager with the Wellspring hospice team, you’ll be part of a team that is committed to delivering exceptional services to seniors and their families in Frankenmuth, Saginaw and the surrounding areas. You must hold a current, valid license in the State of Michigan as a Registered Nurse (RN). Experience in health services management with at least one year of supervisor experience in hospice, home health care, long-term care or a related healthcare program is preferred.
As a Hospice Clinical Manager, you will:

Provide oversight of all patient care services and personnel.

Schedule patient visits with appropriate personnel assignments in collaboration with the scheduler

Coordinate referrals and patient care with multiple disciplines

Assure patient needs are continually assessed & individualized care plans are develop, implemented and revised as necessary

Oversee hospice, MSW, nutritionists and agency services, through orientation activities, consultation, case conferences, clinical in-services, Interdisciplinary Group (IDG) and other meetings

Perform coaching/training visits with staff
- Share in the supervisory extended hour on-call rotation for weekends and after hours (5-6 week rotation)
- Participate in individually assigned committees and associations

Review and ensure accurate, complete comprehensive assessment and completion of client/family record at time of admission, change of condition, resumption of care, in-patient stay, re-certification and discharge

Respond to phone calls from field staff, payers, physicians, clients and families

Chair hospice nurse meetings and case conferences - IDT meetings

Facilitate hospice nurse on-call coverage after hours and on weekends

Provide back-up visits when necessary

Monitors patient and staff incidents, adverse outcomes, patient/family satisfaction and equipment/service problems pertaining to the delivery of patient care service

Perform internal audits and inspections to ensure compliance with organization practices and procedures, State and Federal regulations and clinical practice standards

Respond to ADR requests

Monitor organization compliance with regulatory & professional standards

Complete field visits with clinical staff at least annually and as needed

Schedule patient visits with appropriate personnel assignments in collaboration with patient needs and CMS guidelines

Oversee hospice clinical staff and agency services through orientation activities, consultation, case conferences, clinical in-service, Interdisciplinary Group (IDG) and other meetings as necessary

Provide back-up visits when necessary; Routine and Emergent
- Provide Start of Care visits (Admissions) for new patients, as needed
