Hello! Thank you for your interest in joining the Wellspring team as a part time Hospice Nurse. As a Hospice Nurse with Wellspring, you'll be part of a team of nurses, CNAs, social workers, and administrative professionals who are committed to supporting our staff in their work of serving seniors and their families within the Saginaw, Frankenmuth and surrounding areas. In this position, you must be currently licensed by the State of Michigan Board of Nursing as a Licensed Practical Nurse and successful completion of an accredited Licensed Practical Nursing program. Two years of nursing experience with current home health / hospice, medical, surgical, or critical care experience is preferred.
As a Hospice Nurse you will :
Prepare for visits by reviewing the Plan of Care and current client status reports; receive directions from RN Case Manager and / or Clinical Managers as appropriate
Provide direct client care to clients assigned by the registered nurse and in accordance with physician’s orders and the established plan of care
Identify factors affecting care by evaluating the client’s physical, mental, emotional, environmental and social needs with regard to clinical diagnosis and medical condition
Provide semi-complex treatments and / or nursing procedures within the scope of practice of a licensed practical nurse and agency policies
Administer medications and treatments as prescribed by the physician and monitor the client’s response
Report significant client information and changes in the client’s condition to the RN Case Manager and physician; document communication with the Case Manager after every visit
Assist the client with meeting ADL needs and facilitate the client’s efforts towards independence
Promote a safe environment for the client
Maintain accurate and timely clinical records in compliance with internal, local, state and federal requirements
Maintain expected level of productivity per policy
Participate in (QI / QA) quality improvement process
Licensed Practical Nurse will not be responsible for :
Admit or discharge clients to service
Establish the Plan of Care
Administer any high-tech medications
Conduct Home Health Aide supervisory visits
Complete an OASIS Survey
Start IV lines
Mix and add additives to TPN
Draw blood from an IV line
Supervise or evaluate other employees
