This RN Case Manager position offers a unique opportunity to coordinate and manage patient care across various healthcare settings, ensuring continuity of care, appropriate utilization of resources, and adherence to treatment plans. You will work closely with multidisciplinary teams, advocating for patients' needs and providing education and support to patients and families.
To be successful in this role, you will require:
- A valid Registered Nurse (RN) license in the state of practice.
- Experience in case management, care coordination, or clinical nursing, particularly in medical-surgical, home health, or specialized units.
- Strong communication and interpersonal skills, with the ability to work effectively with multidisciplinary teams.
- Basic Life Support (BLS) certification.
