Role Overview
We are seeking a highly skilled and compassionate Home Health Registered Nurse (RN) to provide high-quality patient-centered care in the comfort of patients' homes. The successful candidate will promote health, manage chronic conditions, and assist with recovery or palliative care.
- Patient Assessments: Conduct comprehensive patient assessments, including physical, emotional, and social needs, to develop individualized care plans that address each patient's unique circumstances.
- Medication Management: Administer prescribed medications, wound care, IV therapy, and other treatments as required, ensuring accuracy and attention to detail.
: Educate patients and families on health conditions, medication management, self-care strategies, and disease prevention techniques to empower them to take an active role in their health. : Continuously monitor patients for changes in health status, responding promptly to acute issues such as infections, medication reactions, or chronic condition exacerbations. : Collaborate with physicians, specialists, and interdisciplinary teams to coordinate care, ensuring continuity and improved health outcomes for our patients. : Provide wound care, dressing changes, catheter care, and other specialized nursing interventions to meet the complex needs of our patients. : Assist patients with mobility, self-care, and activities of daily living (ADLs) when necessary, promoting independence and dignity. : Promote infection control and safety in the home setting to prevent complications and ensure a safe environment for our patients. - <>: Accurately and thoroughly document patient care, progress, and outcomes, ensuring compliance with healthcare regulations and maintaining accurate records.
