Key Responsibilities:

- Collaborate with Healthcare Professionals: Work closely with physicians, therapists, and other healthcare professionals to develop and adjust patient care plans, ensuring seamless coordination of care.
- Monitor and Document Progress: Continuously monitor residents' health status, document vital signs, medication administration, and overall progress, maintaining accurate and up-to-date records.
- Educate Residents and Families: Provide education and support to residents and their families on health conditions, care needs, and treatment options, promoting informed decision-making and empowerment.
- Clinical Expertise: Apply clinical expertise to address acute and chronic conditions, ensure timely interventions, and coordinate care to minimize risks to residents.
- Safety and Compliance: Ensure adherence to all federal, state, and facility regulations related to long-term care, maintain a safe environment, and prevent adverse events.

Qualifications:
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.

- Certification: Basic Life Support (BLS) certification required; Advanced certifications such as ACLS or Geriatric Nursing (e.g., CGRN) preferred.
