Job Title: Acute Rehabilitation / Transitional Care Unit (TCU) Registered Nurse
Job Summary:
An Acute Rehabilitation / Transitional Care Unit (TCU) Registered Nurse provides specialized nursing care to patients recovering from acute illnesses, surgeries, or injuries in an intensive rehabilitation setting.
Key Responsibilities:
- Assess, plan, and provide nursing care to patients in acute rehabilitation and transitional care units recovering from illnesses, surgeries, or injuries.
- Collaborate with multidisciplinary teams, including therapists, social workers, case managers, and physicians, to develop and implement individualized care plans.
- Monitor and manage patients' vital signs, pain levels, mobility, and progress toward rehabilitation goals.
- Administer medications, IV therapies, wound care, and other treatments related to the recovery process.
- Provide education and support to patients and families regarding care plans, medication management, wound care, physical therapy, and self-care strategies.
- Assist patients with daily activities such as mobility, transfers, and personal care, promoting independence and safety.
- Recognize and respond to potential complications such as infections, pain, or mobility issues, implementing interventions as needed.
- Document patient assessments, interventions, and outcomes accurately and in compliance with hospital policies and regulatory standards.
- Facilitate patient transitions from acute care settings to home or outpatient services, collaborating with case managers, social workers, and other team members.
- Participate in interdisciplinary team meetings, contributing input on patient progress, goals, and discharge planning.
Qualifications:
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
- Licensure: Valid Registered Nurse (RN) license in the state of practice.
- Certification: Basic Life Support (BLS) certification required. Advanced Cardiac Life Support (ACLS) and/or Certified Rehabilitation Registered Nurse (CRRN) preferred.
