A Long-Term Care (LTC) Registered Nurse (RN) provides comprehensive patient-centered care to individuals in long-term care facilities such as nursing homes rehabilitation centers and assisted living communities. They are responsible for managing the overall health and well-being of patients ensuring personalized care plans and collaborating with interdisciplinary teams to promote optimal outcomes.
Key Responsibilities:
- Provide direct nursing care to residents in accordance with individualized care plans.
- Administer medications perform treatments and monitor patients' progress ensuring safety and adherence to established protocols.
- Assess plan implement and evaluate nursing care for long-term care residents.
- Collaborate with physicians therapists and other healthcare professionals to develop and adjust patient care plans.
- Monitor and document residents' health status including vital signs medication administration and overall progress.
- Educate residents and their families on health conditions care needs and treatment options.
- Respond to and manage acute and chronic conditions ensuring timely interventions and care coordination.
- Provide emotional support and counseling to residents and their families promoting a positive living environment.
- Ensure compliance with all federal state and facility regulations related to long-term care.
- Supervise and delegate tasks to licensed practical nurses (LPNs) and nursing assistants (CNAs).
- Participate in quality improvement initiatives staff training and continuing education.
- Maintain accurate patient records and documentation adhering to established clinical standards and protocols.
- Address concerns related to infection control safety and preventative care to minimize risks to residents.
Qualifications:
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
- Licensure:
- Valid Registered Nurse (RN) license in the state of practice.
- Certification:
- Basic Life Support (BLS) certification required.
- Advanced certifications such as ACLS or Geriatric Nursing (e.g. CGRN) preferred.
- Experience: Previous experience in long-term care gerontology or related setting
