Key Responsibilities:
• Develop and implement individualized care plans for residents, ensuring personalized and effective care delivery.
• Collaborate with interdisciplinary teams to promote optimal outcomes and improve resident quality of life.
• Educate residents and their families on health conditions, care needs, and treatment options, promoting informed decision-making and self-advocacy.
• Respond to and manage acute and chronic conditions, ensuring timely interventions and care coordination.
Qualifications:
• Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
• Valid Registered Nurse (RN) license in the state of practice.
• Basic Life Support (BLS) certification required. Advanced certifications such as ACLS or Geriatric Nursing (e.g., CGRN) preferred.
