Job Overview:
We are seeking a high energy and passionate Resource Coordinator to support our Residential Services Department. The ideal candidate will join us in our amazing mission of empowering the lives of adults with intellectual and developmental disabilities (STARS). The role of the Resource Coordinator is to maintain a holistic picture/view of the needs of individual STARS and serve as a connector among all teams/stakeholders to meet the needs of the STARS. This should be done throughout our programming, including service plan management within our databases and the Case Management Agency (CMA). The Resource Coordinator embraces the Mission Statement of Wellspring Community: To provide a work, enrichment, and educational community where adults with intellectual and developmental disabilities are empowered to live full, productive, and satisfying lives in a God-centered and nurturing environment.
Essential Duties & Responsibilities:
Including but not limited to the following:
- Manages all aspects of residential services case management/resource coordination for all participants in Unity at Park independent Living Program
- Collaborates with interdisciplinary teams including Case Managers, Wellspring professionals, parents/guardians, therapists/physicians, and behavioral/speech specialists.
- Attends team meetings with case managers for annual service plan renewals, interdisciplinary team (IDT) meetings, SIS level reviews, and other necessary meetings involving STAR support.
- Communicates with private pay families, individual case managers & CMAs (Case Management Agencies)
- Review and process paperwork including monitoring reports, reviewing general IDT notes, and annual review of service plans.
- Analyze progress, coordinate assessments, and develop face sheets.
- Create Individual Service and Support Plans (ISSP’s) for STARS.
- On-call duties as needed.
- Maintain current knowledge, understanding, and application of Rules and Regulation requirements from Federal program, State entities, and local laws that affect service delivery.

- Able to work well with team members in all departments within Wellspring Community and the public at large.
- Well-developed communication skills, both written and verbal.
- Proven ability to manage projects, solve problems and take initiative.
- Demonstrates ability to manage challenging behaviors through the execution of positive behavior supports for each individual/unique situation or seeks help from senior positions when necessary.
- Excellent time management skills and ability to multi-task.
Qualifications & Skills:
- High School Diploma or Equivalent required.
- Bachelor’s degree in social work or related field preferred.
- Proven experience in case management in a residential setting, social services, or a related field is preferred.
- Proficiency in computer skills including Microsoft Office and Electronic Charting Systems.
- Must be able to pass a background check and drug screen and obtain DOT certification.
