Role Overview
The Care Coordinator RN Sr. is responsible for ensuring the smooth transition of patients from acute care to post-acute settings. This position requires strong clinical expertise, excellent communication skills, and the ability to work collaboratively with multidisciplinary teams.
Key Functions
- Assessing patients' transitional care needs and developing individualized discharge plans
- Coordinating post-acute referrals, including home health, hospice, and skilled nursing facilities
- Collaborating with physicians, care teams, and payers to ensure timely and efficient delivery of care
- Identifying and resolving barriers to discharge, including delays in diagnostic or treatment plans
Required Skills and Qualifications
- Bachelor's degree in Nursing
- Current RN licensure in a single state or multi-state compact
- Basic Life Support certification
- Preferred: Certified Case Manager (CCM) certification
