About the Role
We are seeking a highly skilled and experienced Medical Care Coordinator Specialist to join our team at Wellstar Health System, Inc. As a key member of our care coordination team, you will be responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patients and their families to ensure that their care needs are met.
Key Responsibilities
- Assessing transitional care needs and developing individualized care plans
- Coordinating care across the continuum, including hospital, post-acute, and community-based services
- Engaging with patients and their families to educate them on their care plans and facilitate timely discharges
- Collaborating with healthcare providers and other members of the care team to ensure seamless transitions of care
Requirements
- Bachelor's degree in Nursing or related field
- Minimum 3 years of experience as a staff nurse in an acute care hospital setting
- Minimum 2 years of experience as a case manager in a hospital setting or payer-based model
- Valid RN license in the state of practice
