About the Role
This position is responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure their care needs are met. The Care Coordinator RN Sr. requires strong clinical expertise to partner with physicians and care teams to drive optimal and efficient treatment plans.
Key Responsibilities
- Assessing transitional care needs and developing individualized discharge plans
- Coordinating care across the continuum, including post-acute referrals and patient/family education
- Collaborating with physicians, care teams, and payers to ensure timely and efficient delivery of care
- Identifying and resolving barriers to discharge, including delays in diagnostic or treatment plans
Requirements
- Bachelor's degree in Nursing
- Current RN licensure in a single state or multi-state compact
- Basic Life Support certification
- Preferred: Certified Case Manager (CCM) certification
