Key Responsibilities:
- Assess patients' transitional care needs and develop personalized discharge plans in collaboration with interdisciplinary teams
- Communicate effectively with patients, families, and healthcare providers to ensure timely and efficient transitions of care
- Proactively identify delays or obstacles in diagnostic or treatment plans, escalating issues as necessary to facilitate resolution
- Collaborate with social workers, home health agencies, and other stakeholders to coordinate post-acute care services
- Participate in quality improvement initiatives to enhance patient satisfaction and reduce readmissions
Requirements:
- Bachelor's degree in Nursing (BSN)
- Current licensure as a Registered Nurse in the state of Georgia (or eligibility for compact license)
- Minimum 3 years of experience as a staff nurse in an acute care hospital setting, with at least 2 years of experience in case management or similar roles
- Excellent written and verbal communication skills, with the ability to adapt to diverse communication styles and preferences
- Strong analytical and problem-solving skills, with the capacity to think critically and prioritize tasks effectively
- Ability to work collaboratively with multidisciplinary teams and adapt to changing priorities and deadlines
