We are seeking a skilled Pharmaceutical Care Pharmacist to integrate patient-centered care with distributive activities. The ideal candidate will provide consultative services, including antibiotic dosing and medication therapy advice, while adhering to Georgia State Pharmacy Law.
Core Responsibilities and Essential Functions:
General Pharmacy Practice
* Preparing IV Admixtures and compounding pharmaceuticals
* Entering orders into the pharmacy computer system and dispensing medications
* Following patients for antibiotic and anticoagulant dosing
* Participating in daily rounds and educating patients on new medications and side effects
Knowledge of Drug Appropriateness
* Evaluating and monitoring drug regimens for appropriateness
* Interpreting physician orders and screening for interactions
* Providing patient education and reviewing medication profiles
Required Minimum Education:
- Bachelors or Doctorate in Pharmacy (Preferred)
Required Minimum License(s) and Certification(s):
- Registered Pharmacist
Additional License(s) and Certification(s):
- Board Certified Pharmacotherapy Specialist (Preferred)
Required Minimum Experience:
- Hospital pharmacy experience (Preferred)
- Minimum 2 years clinical pharmacy experience (Preferred)
- Completion of PGY1 Pharmacy Residency (Preferred)
Required Minimum Skills:
- Strong order entry and computer skills
- Good oral communication skills
- Ability to think/act independently
- Collaboration with other professionals to ensure good patient experience
