What are the responsibilities and job description for the Bridge Team Case Manager position at WellStone?
Job Description : The Bridge Team Case Manager assists clients who struggle with varying mental health issues. The goal of this position is to guide the client towards psychiatric stability and help them to develop healthy basic living skills. The case manager will have to conducts assessments, completes ISPs, visits clients to assess progress, and serve as a case manager as part of a treatment team. Must be willing to manage a caseload of 15-20 people and will have to conduct weekly visits. Provide Crisis Intervention and Resolution as required to include non-traditional hours.
What we're looking for :
Bachelor's degree in social sciences field
Excellent 5 year driving record & reliable transportation
Excellent verbal and written communication skills
Personal Automobile Liability Insurance in the amounts of $100k, $300k, $50K
What you'll be doing :
Visit clients in their environment to assess progress towards goals and provide support
Primary caseload will consist of 15 to 20 clients.
Understand the outpatient commitment process
Act as auxiliary provider for hospital tracking for Bridge Team
Act as auxiliary provider for outreach programs as necessary
Facilitate individual / group basic living skill training sessions per SUN-R assessment
Assist with finding affordable and stable housing for homeless clients
Maintain knowledge of community resources and link to assist clients / family system
Effectively communicate clients' needs to treatment team
Facilitate SUN-R assessments at admission and every 6 months
Facilitate ISP at admission and every 6 months per WellStone policy
Assist / Coordinate clients' community appointments and center
Document services within 24 hours per center's policy
Demonstrate an understanding of Case Management interventions per ADMH, Medicaid Codes
Address and follow up on client crisis
Attend homeless initiatives : Zero Fee site visits, Soup Kitchen, community meetings, HMIS data
Assist and link clients to resources in their community
Assist with initiating disability benefits utilizing SOAR process
Develop and maintain good communication with stakeholders
Complete other duties as assigned by supervisor
