Job Description:
WellTech Partners is seeking a compassionate and dedicated Social Worker to join our team at. As a Social Worker, you will play a crucial role in enhancing the well-being and quality of life of our residents by addressing their medical, emotional, social, and psychological needs. Responsibilities:
- Provide psychosocial support to help residents cope with the transition to nursing home life, addressing issues such as depression, anxiety, and feelings of isolation.
- Advocate for residents' rights and needs, ensuring they are respected and met.
Pl- Develop and implement individualized care plans in collaboration with the healthcare team.
- Offer support and counseling to residents' families, helping them understand the needs and conditions of their loved ones.
- Connect residents and families with community resources, including financial assistance, transportation, and support groups.
- Contribute to improving residents' quality of life by addressing their social, emotional, and psychosocial needs.
Qualifications:
- A minimum of a bachelor’s degree in social work or a related human services field.
- At least 2 years of supervised social work experience in a healthcare setting, working directly with individuals.
- Bilingual proficiency in Spanish and English.
- Strong interpersonal and communication skills.
- Ability to work collaboratively as part of a team.
- Knowledge of mental health laws and regulations.
