What are the responsibilities and job description for the Community Health Worker position at WellWise Services?
Duties include:
- Attend in-home participant meetings and other community based appointments.
- Conduct comprehensive assessments for non-waiver participants.
- Work with family members and other volunteer caregivers to maximize available services to the participant.
- Develop a plan of care identifying needed services, supports, and interventions in collaboration with a licensed staff member.
- Arrange and coordinate delivery of needed services (utilizing natural supports and community resources).
- Monitor participant Person Centered Service Plan to assure delivery of needed services.
- Evaluate and modify Person Centered Service Plan as required.
- Assist other care managers with questions resulting from assessments and other situations.
- Participate in regular staff meeting and training events.
- Participate in community events to teach, assist, advocate, and forge community partnerships to manage needs not addressed by natural supports or in-home service providers.
- Perform other duties as assigned.
Qualifications:
- Minimum of an Associate's degree in a social services field is preferred. Will consider relevant experience.
- Minimum of two years Care Manager Experience preferred.
- Experience in the health care system and the field of aging.
- Scheduling experience.
- Strong computer skills/proficiency with Microsoft Office products.
- Community Health Worker Certification preferred.
Full Time: 40 hours per week. Work schedule M-F excluding observed holidays. Union position. $20.97/hour. Benefits include paid time off and holidays, health insurance and employer contribution to a 403(b) retirement account.
