The Spiritual Care Coordinator / Chaplain is a professional position responsible for the provision of spiritual care services to all WDH patients, families, and caregivers, acknowledging and respecting their beliefs, culture, and values. Our mission is to provide the best care for every person. We strive to give customized holistic care to individuals with chronic diseases and conditions as well as those who need short-term care. The Spiritual Care Coordinator / Chaplain will use his/her theological and clinical training to help patients and family(s) utilize beliefs and practices as coping resources. Chaplaincy care is grounded in initiating, developing, and deepening, and bring to an appropriate close, a mutual empathetic relationship with patient, family and/or staff. After assessing the patient's spiritual practices and values, they can help communicate these values to the patient's interdisciplinary health care team. The chaplain will work as a team member in accordance with the Hospital, Chaplaincy, and Palliative Care Missions and Vision statements. Experience Minimum Required:

One year of related work experience required. - Experience with computer systems required, including web-based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access. - Possess Excellent communication and human relation skills. - Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation, and ability to work effectively within an interdisciplinary team.

Experience Preferred/Desired: Two years or more of hospital chaplaincy - Demonstrated experience in working with patients/families dealing with life threatening illness and death. - Experience with database software or electronic medical record software.

Education Minimum Required:

Bachelor's degree from an accredited CHEA - Certification: 4 or more CPE Units master's degree in theology, Philosophy or Psychology.

Education Preferred/Desired - Additional units of CPE. - Training Minimum Required - Commission to function in a ministerial capacity by a recognized faith group for ministry in a specialized setting. - Training Preferred/Desired - Board Certification APC, NACC, NAJC

Demonstrates competency to act as a liaison between the hospital and community clergy, lay ministers and congregations.

Utilizes appropriate spiritual/religious community resources and serves as a liaison between WDH and the community clergy directly involved with the patient/family.

Demonstrates competency in assisting in the assessment of spiritual needs of patients and families and in providing spiritual care

The chaplain will perform both independent visits with families as well as joint visits with clinicians when possible and beneficial.

Provides chaplaincy services to all patients and families to optimize quality of life by anticipating, preventing, and treating suffering throughout the continuum of care. When needed, the chaplain will participate in family meetings as necessary.

Provides timely and appropriate documentation in patient's medical record.

Presents and/or discusses clinical/pastoral work in formal and informal settings. Formulateand utilize spiritual assessments, interventions, outcomes, and care plans to contribute effectively to the well-being of the person receiving care.

Provides spiritual counseling and services, including crisis intervention, to meet the psycho-spiritual, religious, and existential needs of patient/family in collaboration with the IDT.

Demonstrates competency in maintaining a spiritual presence within the hospital

Considered part of the Interdisciplinary team, including Palliative care.

Collaborates with Interdisciplinary teams with a focusing on patients and family.

Chaplaincy services address personal, spiritual, emotional, and psychosocial issues.

Provides consultation and education to team on patient-family management as appropriate.

Participates in supervision as appropriate to level of professional development. Serves as a mentor and/or training resource to less experienced staff. Supports environment of idea sharing and feedback. Seeks opportunities and sets goals for own professional development. Identifies and learns from mentors.

Provide spiritual care that respects diversity and differences including, but not limited to culture, gender, sexual orientation, and spiritual/religious practices.

Participates in the assessment, education and resolution of medical ethical issues.

Assists inter-disciplinary team members in understanding the significant psycho-spiritual, religious and existential factors related to the patient/family's quality of life and in developing support for patient/family with regard to spiritual care.

Attends patient care conference and group meetings as a member of the interdisciplinary group.

Employee must be FIT tested for an N95 mask and participate in competency for Donning and Doffing.
