Pay Rate: $21.00 per hour
Job Description: Memory Care Program Assistants for our All Points team will provide support to residents living with dementia in a person-centered methodology. They will be responsible for documenting and assessing interactions, behaviors, and progression of dementia, as well as facilitating one-on-one and group activities for individuals living with dementia. Effective communication with the care team is crucial to maintaining a high quality of life for all residents.
Responsibilities:
- Provide person-centered support to residents living with dementia, in accordance with individual care plans.
- Document and assess interactions, behaviors, and progression of dementia.
- Communicate effectively with the care team to ensure a high quality of life for residents with dementia.
- Facilitate one-on-one and group activities for individuals living with dementia.
Requirements
- High school diploma or equivalent required, associate's or bachelor's degree is preferred.
- Prior experience in dementia care is preferred.
- Experience in supporting individuals with disabilities is preferred.
