Job Title: Case Manager-Philly Home
Work schedule: Monday through Friday from 9:00am to 5:00pm.
GENERAL OVERVIEW OF KEY ROLES AND RESPONSIBILITIES:
The individual occupying this position will assist Philly Home Staff in gaining access-needed resources such as medical, social, educational, housing, and any other needed supportive services. This position will report directly to and supervised by their assigned Philly Home Director. These functions are directed at specific outcomes for the benefits of the persons served and should follow the priorities listed below in assisting residents with meeting their needs;
1. Organizing and bringing to bear the natural supports available to the person through family, friends, neighbors, etc. when residents needs cannot be met through these means;
2. Organizing and bringing to bear resources available in the local community and generic service providers. When the residents' needs cannot be met through these means, then
3. Obtaining specialized services through available mental health treatment and other rehabilitation services. This includes service providers who specialize substance treatment under DBHIDS for prevention and harm reduction.
ESSENTIAL & CORE FUNCTIONS:
1. Advocacy efforts to improve services, eliminate stigma, resolve problems, etc.
2. Support network building.
3. Assists the residents in identifying, accessing and learning to use community resources appropriately to meet his/her daily living needs.
4. Referral and linking with appropriate services as specified in the treatment/recovery plan.
5. Provide participants with community-based skill training.
6. Required use of public transportation and use of agency vehicle when performing community activities with program participants.
7. Provide case management services in accordance with written, participant specific, recovery plan that assesses strengths, needs, interests; and is goal outcome oriented.
8. Provide case management and intervention services as needed where the participant resides or may need services within and outside of the Philly Home Program.
9. Provide crisis intervention services to all Philly Home residents.
10. On call pager or cell phone rotation to handle all emergency/crisis intervention services between 5:00pm and 8:00am. Expected response time is one hour before 9:00pm and eight hours after 9:00pm.
11. Flexible work schedule when necessary.
12. Written case notes documenting the purpose of each contact, recovery goals; crisis intervention; date and time service was provided.
13. Participation in individual and/or group supervisory meetings.
14. Participation in treatment and recovery planning meetings.
15. Participation in staff or DBH/MR approved training. Mandatory WES training.
ADDITIONAL RESPONSIBILITIES:
1. Performs other duties and special projects as assigned.
PREREQUISITES & QUALIFICATIONS FOR THE POSITION:
1. Bachelor's degree in or a Health and Human Services/Administration Degree and have a minimum of one (1) year of case management or counseling experience with adults and or families diagnosed with mental illness.
2. Valid FBI clearance, criminal history check and child abuse history clearance required.
3. Participation in and completion of all CBH mandatory in-services (must be completed with three (3) months of hire and then annually from the date of the initial training).
4. Valid Pennsylvania Driver’s License.
