& Hospice role. We are seeking an experienced and mission-driven Clinical Director to oversee and
manage the Home Health & Hospice
clinical operations in Washington State .
The Clinical Director is responsible for ensuring high-quality patient care, compliance with Washington State Department
of Health (DOH), Medicare, and Medicaid regulations , and leading a team
of healthcare professionals. This role requires expertise in regulatory compliance, leadership,

You will
provide continuing care through these responsibilities
Essential
functions of this position include the following.
Clinical
Oversight & Quality Assurance
- Ensure
all home health and hospice services comply with Washington State Department of Health (DOH) licensing regulations .
- Maintain
adherence to Medicare Conditions
of Participation (CoPs), Medicaid requirements, and accreditation
standards (Joint Commission, CHAP, ACHC).
- Develop,
implement, and oversee Quality
Assurance and Performance Improvement (QAPI) programs .
- Monitor
and improve patient outcomes,
safety measures, and adherence to clinical best practices .
- Stay
updated on Washington State laws
regarding opioid prescribing, advance directives, and end-of-life care .
Leadership
& Staff Development
- Supervise,
mentor, and support nurses,
therapists, social workers, and home health and hospice aides .
- Ensure staff training, competency evaluations,
and continuing education (CE) compliance per Washington State
requirements.
- Conduct interdisciplinary team meetings, case
reviews, and staff performance evaluations .
- Build a
culture of belonging through active use of the company’s recognition and

- Create
an engaged and high-performing team through adoption and visual support of
the company’s people strategies.
Regulatory
Compliance & Documentation
- Oversee documentation practices to ensure
compliance with Washington State
DOH, Medicare, and Medicaid standards .
- Ensure
agency readiness for state
surveys, federal audits, and accreditation inspections .
- Maintain
compliance with Washington State’s
POLST (Physician Orders for Life-Sustaining Treatment), Death with Dignity
Act, and Medicaid waiver programs .
- Address patient complaints, incidents, and
grievances while ensuring proper reporting procedures.
Operational
& Financial Management
- Work
with the Executive Director,
Associate Director, and finance team to manage clinical budgets,
resource allocation, and cost-effective care delivery.
- Optimize
staffing and scheduling to ensure efficiency while maintaining
high-quality patient care.
- Oversee
utilization management to ensure appropriate
and ethical use of Medicare/Medicaid reimbursements .
- Develop
a plan for continued growth of census and services.
Patient
& Family Advocacy
- Serve as
a liaison between patients,
families, physicians, and interdisciplinary teams to ensure
holistic and compassionate care.
- Educate
patients and families on hospice
eligibility, disease progression, and palliative care options .
- Coordinate
with community organizations,
volunteer programs, and support services to enhance patient care.
- Leverage
Lean thinking and methodology to develop contingency plans and staff
competency with service recovery.
Our requirements and qualifications for success
- Active, unrestricted RN (Registered Nurse) license in Washington
State (WA DOH licensed) or out-of-state MSL.
o Follow the Washington Nurse Practice Act.
o If MSL, complete Suicide Prevention Training in advance of hire
per state regulations.
o If MSL, complete Washington State Board of Nursing (WABON) MSL
demographic survey in advance of hire per state regulations.
- Bachelor’s degree in Nursing (BSN) required; Master’s degree (MSN,
Healthcare Administration, or related field) preferred.
- Minimum of 3-5 years of leadership experience in home health
and/or hospice care.
- Deep knowledge of Washington State DOH home health and hospice
regulations, Medicare/Medicaid CoPs, and accreditation standards.
- Strong leadership, problem-solving, and organizational skills.
- Proficiency in Electronic Health Records (EHR), case management,
and clinical reporting systems.
- Ability to lead interdisciplinary teams, drive performance
improvement, and ensure regulatory compliance.
- Experience managing hospice and home health surveys, audits, and
accreditation processes.
- Bilingual skills; preferred but not required.
- Certification in Hospice & Palliative Nursing (CHPN) or Home
Health Compliance (HCS-D, COS-C, etc.); preferred but not required.
Physical,
environmental, and mental requirements
- Ability to give direct patient care, including participating in
lifting, transferring, positioning, and assistance of residents with walking or
in wheelchairs.
- Able to lift 40lbs on a regular basis.
- Ability to use a laptop, tablet or touch screen to document
resident records.
- Apply your physical and muscle strength to lift, push, pull or
carry objects.
- Ability to utilize your lower back and abdominal muscles to
support part of the body repeatedly and continuously over time without
fatiguing.
- Ability to listen to and comprehend information and ideas provided
through conversation, email, and text.
- Ability to maintain focus and concentration.

Salary Range: $104,665.60 to $158,662.40 per year
