Clinical Manager, Home Health & Hospice role. We are
seeking a dedicated licensed Clinical
Manager to oversee and coordinate the daily clinical operations of our Home Health & Hospice programs in Washington State . The licensed Clinical
Manager is responsible for ensuring high-quality
patient care, compliance with state and federal regulations, and efficient
management of clinical staff . This position plays a key role in
maintaining DOH (Washington State
Department of Health) compliance, Medicare/Medicaid regulations, and
accreditation standards while fostering a culture of excellence,

You will
provide continuing care through these responsibilities
Essential
functions of this position include the following.
Clinical
Oversight & Quality Assurance
- Ensure
all patient care services adhere to Washington State Department of Health (DOH) home care and hospice
licensing regulations .
- Maintain
compliance with Medicare/Medicaid
Conditions of Participation (CoPs) and accreditation standards (Joint
Commission, CHAP, or ACHC).
- Oversee patient assessments, care planning, and
documentation to ensure accuracy, quality, and compliance.
- Implement
and monitor quality assurance and
performance improvement (QAPI) programs to enhance patient
outcomes.
- Stay
current with Washington State’s
opioid prescribing laws, advanced directives regulations, and end-of-life
care policies .
Team
Leadership & Staff Management
- Supervise
and support RNs, LPNs, therapists,
social workers, chaplains, bereavement staff, volunteers, and home health and
hospice aides .
- Conduct staff training and competency
evaluations per Washington State DOH guidelines.
- Lead case conferences and interdisciplinary
team meetings to promote continuity of care.
- Ensure
compliance with Washington’s
continuing education (CE) requirements for nursing professionals .
- Build a

- Create
an engaged and high-performing team through adoption and visual support of

Regulatory
Compliance & Documentation
- Ensure timely, accurate, and complete
documentation in Electronic
Health Records (EHR) in compliance with Washington State home
health & hospice documentation standards.
- Prepare
for state DOH inspections, Medicare
audits, and accreditation surveys .
- Maintain
compliance with Washington’s POLST
(Physician Orders for Life-Sustaining Treatment) and Advance Directive
requirements .
- Address
and resolve patient grievances and
complaints in accordance with Washington State Patient Rights
regulations.
Operational
& Financial Efficiency
- Manage staff scheduling and resource allocation
to optimize care delivery while maintaining cost efficiency.
- Work
with leadership to ensure financial
sustainability and proper utilization of Medicare and Medicaid
reimbursements under Washington State guidelines.
- Assist
in the development of policies and
procedures specific to home health and hospice services in Washington .
Patient
& Family Advocacy
- Serve as
a liaison between patients, families,
caregivers, and community resources to ensure comprehensive
support.
- Educate
patients and families on Washington’s
Death with Dignity Act , hospice eligibility criteria, and
palliative care options.
- Promote community outreach, patient education,
and end-of-life planning initiatives .
- Leverage
Lean thinking and methodology to develop contingency plans and staff
competency with service recovery.
Our requirements and qualifications for success
- Active, unrestricted RN (Registered Nurse) license in Washington
State (WA DOH licensed) or out-of-state MSL.
o Follow the Washington Nurse Practice Act.
o If MSL, complete Suicide Prevention Training in advance of hire
per state regulations.
o If MSL, complete Washington State Board of Nursing (WABON) MSL
demographic survey in advance of hire per state regulations.
- Bachelor’s degree in Nursing (BSN) required; Master’s degree (MSN,
Healthcare Administration, or related field) preferred.
- Minimum of 2-4 years of experience in home health, hospice, or a
related healthcare field.
- Strong knowledge of Washington State DOH home health and hospice
regulations, Medicare/Medicaid requirements, and accreditation standards.
- Leadership experience with staff supervision, team management, and
performance evaluations.
- Proficiency in Electronic Health Records (EHR) and case management
software.
- Strong organizational, problem-solving, and decision-making
skills.
- Excellent communication and interpersonal skills with a focus on
patient-centered care.
- Experience with survey readiness, quality assurance, and
performance improvement programs.
- Knowledge of Washington Medicaid Home and Community-Based Services
(HCBS) waiver programs.
- Certification in Hospice & Palliative Nursing (CHPN), OASIS
(Outcome and Assessment Information Set), or other relevant credentials.
- Bilingual skills; preferred but not required.
Physical,
environmental, and mental requirements
- Ability to give direct patient care, including participating in
lifting, transferring, positioning, and assistance of residents with walking or
in wheelchairs.
- Able to lift 40lbs on a regular basis.
- Ability to use a laptop, tablet or touch screen to document
resident records.
- Apply your physical and muscle strength to lift, push, pull or
carry objects.
- Ability to utilize your lower back and abdominal muscles to
support part of the body repeatedly and continuously over time without
fatiguing.
- Ability to listen to and comprehend information and ideas provided
through conversation, email, and text.
- Ability to maintain focus and concentration.
