What are the responsibilities and job description for the Assisted Living Activity Coordinator position at Wesley Commons?
Purpose: To provide meaningful, stimulating, entertaining activities to all Health and Rehabilitation Center (HRC) residents in an effort to promote social interaction and provide psychological well-being. Responsible for planning and directing all day to day responsibilities of the HRC Activitys volunteers and Activity Assistant.
Essential Functions:
- Plan, conduct, and direct all daily, weekly and monthly activities and special events for the Health and Rehabilitation Center.
- Coordinate and supervise the daily activities of the Activity Assistant.
- Plan and implement programs to outside sources inviting them to visit facility and provide entertainment including intergenerational activities.
- Provide opportunities for pleasurable external outings based on the needs and abilities of the residents at least monthly.
- Assist in visiting residents on a weekly 1:1 basis to assess their activity needs and plan and conduct reminiscence sessions with residents and families, if necessary. Additionally, documentation must be provided for 1:1 activities. All residents on isolation precautions must have documented / care planned 1:1 activities.
- Maintain activity storage area in a clean, neat, orderly, attractive and stimulating environment as well as ensuring that all items are stored appropriately when not in use.
- Maintain adequate supplies, within budget, for a wide variety of activities including, but not limited to, crafts, games, music, movies, baking, etc.
- Gather materials through research, planning, books and other sources for resident activities.
- Maintain records of activities, attendance and participation in patients charts as required by DHEC. Activity notes must be updated quarterly.
- Responsible for assuring that the activity function of the MDS and the care plan process for the residents is completed timely.
- Responsible for planning, preparing, and maintaining fiscal year budget for the Activity Department.
- Publish, copy and distribute weekly newsletter to inform residents of upcoming events.
- Conduct regular staff meetings with employees.
- Maintain assessment records of each resident and observe cognitive actions and document.
- Prepare monthly activity calendar for the Health and Rehabilitation Center and responsible for ensuring that it is uploaded to our computer system for the digital displays in each wing.
- Serve as back up to the Social Worker for managing residents petty cash funds; mail quarterly statements to POAs and/or residents and maintain documentation.
- Serve as back up to the Social Worker for scheduling and running monthly residents council meeting; type and maintain meeting minutes; ensure follow-up from all departments regarding any issues of concern raised.
- Attend care plan meetings for the Health and Rehabilitation Center residents and document and follow-up as needed. Documentation in Activities and social notes must be maintained.
- Document initial activity admitting note and quarterly assessments; provide visits to residents for support and socialization and document any behavior changes; document any medicine increases or decrease.
- Interview all residents on a quarterly basis, using the MDS questionnaire for section F; Enter data from the MDS questionnaire into the SOS system in a timely manner; updates care plans accordingly and in a timely manner. SOS data is to be entered : 1) Upon admission into the facility 2) Quarterly 3) Re-admission from the hospital 4) Significant change 5) Upon discharge
- Communicate with residents and families to learn residents likes/dislikes, work history, educational background, culture or family experiences; Be aware of residents health profile which may limit certain activities (check with CNA, nurse or physician if needed). Additionally, must attend the pre-admission conference to ensure social needs are met upon admission to the Health Care Center.
- Attend in-services both on campus and off campus, if applicable, dealing with dementia and/or any other job-related seminars.
- Attend , Quality Metric, DHEC meeting and rehab meeting as scheduled
- Ensure weekend and evening activities are provided.
- Responsible for supervising students, interns and volunteers in compliance with company policies and procedures.
- Serve as back up to the Social Worker for documentation of initial social service admitting note and quarterly assessments; provide visits to residents for support and socialization and document any behavior changes; document any medicine increases or decrease.
- Adhere to all HIPAA guidelines and regulations.
- Oversee beautician needs and beauty shop maintenance.
Non-Essential Functions:
- Design and promote stimulating atmosphere and programs.
- Consult with residents that become disoriented or confused, and provide emotional support to residents and notify nursing staff of concerns.
- Maintain CPR certification.
- Special assignments as requested.
Qualifications:
- Prefer Activity Director Certification from an accredited school or 3-5 years experience in a social or recreation program in a health care setting or a degree with an emphasis in health care.
- Must have the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, procedures of the department.
- Must have ability to deal with confidentiality concerning residents medical histories and personal feelings
- Excellent verbal and written communication, interpersonal, problem solving and planning skills to effectively deal with people at different organizational levels both inside and outside the company.
- Must be self-motivated with strong leadership skills.
- Highly detail oriented and organized in work.
- Ability to multitask and work independently with minimal daily direction from manager to accomplish objectives.
- Ability to meet assigned deadlines.
- Excellent interpersonal, communication and problem solving skills.
- Proficient in Microsoft Excel and Word.
