Description

The Social Worker will assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional, and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by the Director of Nursing. 

Responsibilities include but are not limited to:

  • Assist in planning, developing, organizing, implementing, and evaluating the social services programs of this facility.
  • Assist in the development, administering, and coordinating of department policies and procedures.
  • Assist in the developing and implementing policies and procedures for the identification of medically related social and emotional needs of the resident.
  • Responsible for monthly Resident Council Meetings and follow ups from the Meeting
  • Responsible for Issues Of Concern for the building including logging, tracking, and follow up, and reporting results to the management team.
  • Responsible for attending weekly care plan meetings and following up on any concerns or missing personal items.
  • Responsible for the completion of 1718’s through CLTC for Medicaid conversions.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
  • Serves as back up to the Admissions Coordinator to ensure continuity of the facility’s admission process.
  • Assists residents and/or family members with the Medicaid application process.
  • Participate in discharge planning, development and implementation of social care plans and resident assessments. Assist the residents with accessing inpatient, outpatient, extended care, and home health services in the community and making proper arrangements prior to discharge.
  • Responsible for the fiscal management of resident petty cash funds upon written request from the resident if competent and/or the responsible party.  Responsible for maintaining an accurate account of resident’s personal monies/petty cash and written evidence of purchases by the facility on behalf of the residents with written authorization for what items or services were to be purchased.
  • Responsible for providing quarterly, a written report of the balance of the resident’s personal monies/petty cash regardless of a zero balance.
  • Responsible for coordinating a final written notice of the account from finance within sixty (60) days of a resident’s death or discharge. Work with Finance for reimbursement of any personal monies/petty cash due to the resident.
  • Responsible for the scheduling of the care plan meetings, notifying all residents and family members of the care plan meeting’s date and time and documenting in the social service section of the chart.
  •  Interview resident or family members to obtain social history.
  • Involve the resident/family in planning social service programs when possible.
  • Refer resident/families to appropriate social service agencies when the facility does not provide the specific services or needs required by the resident.
  • Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
  • Coordinate social services activities with other departments as necessary.
  • Evaluate social and family information and assist in determining plans for social treatment.
  • Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.
  • Participate in facility surveys (inspections) made by authorized government agencies as necessary.
  • Ensure that all charted progress notes are complete, informative and descriptive of the services provided and of the resident's response to the service.
  • Develop and maintain a good working rapport with other departments within the facility to assure that daily social services activities can be performed without interruption.
  • Assist in creating and maintaining an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility.
  • Serve on, participate in, and attend various committees of the facility as required.
  • Assist in the development of and participate in regularly scheduled orientation and in-service training programs that relate to the social services department.
  • Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
  • Make weekly inspections of all social service functions to assure that quality control measures are continually maintained.
  • Perform other related assignments as required.

Qualifications


Qualifications:

Bachelor’s Degree from an approved school of Social Work along with one year supervised social work experience in a health care setting working directly with individuals. 

  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must possess the ability to make independent decisions when circumstances warrant.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must possess leadership ability and the willingness to work harmoniously with others.
  • Must have patience, tact, cheerful disposition, and enthusiasm.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services.
  • Must be able to relate information concerning a resident’s condition.
