Asa member of the Hospice and Palliative Interdisciplinary Team, the Palliative / Hospice RNCase Manager implements and coordinates care plans from admission to dischargefor terminally ill patients that provide the care and services to treatdistressing symptoms and improve comfort. This role educates families and caregivers on recognizing symptoms andoffering emotional and practical support.
