  • Employees, and their families if elected, can participate in medical and vision insurance (full time and ACA eligible), dental (full time) and group life (employee only for full time, excludes part-time and on-call staff).
  • We offer a combined paid time off (PTO) policy which incorporates state paid sick leave with company paid time off at an accrual rate of 0.0607 per hour worked, equivalent to 120 hours of PTO at 2,080 hours worked in a year. We also offer 6 Paid Holidays (8 hours for full time and 6 hours for part time per event, excludes on-call staff) and 1 Personal Holiday of Choice per year (excludes on-call staff).
  • Retirement planning is encouraged through our 403(b) plan that includes a generous 100% company match on the first 4% of earnings an employee contributes. There is a 5-year vesting schedule on the company match, and minors are ineligible for the company match.
  • A food and beverage discount of 50% is available to all employees at any Wesley bistro. Employees are also eligible for On-Demand Pay with Dayforce Wallet. Minors need parental consent to access this benefit.
  • To assist employees with challenges outside of the workplace, Wesley offers an Employee Assistance Program (EAP), which is 100% company paid. Additionally, Wesley Community Foundation provides grants to qualifying employees as detailed in the plan summary.

Enrich the lives of older adults throughcommunity, choice, and continuing care in the Social Worker role. The Social Worker provides support and servicesto address the social and emotional needs of the client to achieve the goals withinthe plan of care. Services are provided under the direction of a physician andin conjunction with other health care team members in accordance with company policiesand practices. Youwill provide continuing care through these responsibilities

Essential functions of this position include the following.

  • Evaluates and records the care needs of the patient during eachvisit.
  • Communicates with and regularly updates the patient regardingtheir plan of care.
  • Reports observations of the patient's condition to the designatedsupervisor as appropriate.
  • Collaborates with other service providers to discuss the patientplan of care and frequency of visits.
  • Perform both in person and remote visits with patients accordingto Medicare guidelines and company policy.
  • Prepares a social history and plan of care with the purpose ofimproving the scope and quality of existing services or extending services.
  • Assist the patient and their family to utilize existing resourcesin the home environment.
  • Refers patient and family to community agencies and resources; actsas an intermediary for the patient as needed.
  • Assesses when appropriate, a family's financial situation takinginto consideration the patient's prognosis and medical needs; facilitating thereferral process to any agency for financial assistance.
  • Consults with the case manager and members of the health team forthe purpose of assisting them to understand significant social, emotional andenvironmental factors related to the patient's health problems.
  • Plans for patient's discharge, prepares discharge summaries andinstructions.
  • Adhere to HIPAA regulations and ensures confidentiality of patientrecords and information.
  • Attends meetings to enhance team communication and coordination ofservices and care.
  • Complete assigned training and maintain continuing education.
  • Observes, records, and reports signs and symptoms; reactions totreatments and medications; and any change in the patient's physical oremotional condition in a timely basis
  • Records clinical and progress notes at the time of the visit toensure continuity of qualitative patient care on a timely basis.
  • Completes daily report sheets and any required paperwork in atimely fashion to ensure agency operation schedules.
  • Works with patient Long Term Care for more in depth discussionsalongside billing department
  • Generates recertification of the physician plan of care, ensuringtimeliness and quality of documentation.
  • Completes clinical documentation accurately.
  • Other duties and special projects as assigned.
  • Our requirements and qualifications for success

  • Master of Social Work (MSW) degree from a school accredited by theCouncil on Social Work Education.
  • Minimum of one (1) years' experience in a health care setting orequivalent experience. Home health experience preferred.
  • CPR certification, as applicable.
  • Demonstrates excellent written and verbal communication skills andstrong interpersonal skills.
  • Demonstrates knowledge of resources available in the community.
  • A current and valid Washington State driver's license and validvehicle insurance.
  • Physical,environmental, and mental requirements

  • Giving full attention to what other people are saying, taking timeto understand points being made, asking questions as appropriate
  • Near and far vision acuity
  • Ability to sit, stand, bend and twist for prolonged periods orintermittently throughout shift
  • Ability to lift up to 25 pounds
  • Subject to frequent interruptions
  • Minimal, local, same-day travel as needed
