We are currently looking for a full-time Licensed Vocational Nurse (LVN) with a current, active license in the state of Texas to work either the 2:00PM-10:00PM shift within our Skilled Nursing and Rehab Community at The Wesleyan. The LVN is responsible for the care of resident's needs as identified through resident assessments and as described in the resident plan of care, for the overall nursing care of all residents in assigned unit, and for providing professional nursing care to residents while following established facility policies and procedures. A valid, active license as an LVN within the state of Texas along with excellent nursing skills and care of residents, proven indirect leadership skills of other nursing staff, and a heart for serving residents within a skilled nursing community are essential to your success as an LVN at The Wesleyan.

Essential Job Functions:
- Assess, evaluate, plan, and implement resident care plans and respond to resident needs.
- Administer or supervise the preparation and administration of prescribed medications and/or prescribed treatments.
- Supervise the serving of prescribed diets and fluid intake and report persistent unresolved problems to the physician and the Director of Nursing.
- Ensure that all medications and treatments are charted appropriately by the person administering the medication or completing the treatment and ensure appropriate documentation of any discrepancies.
- Order supplies and equipment as needed.
- Oversee and delegate responsibilities as needed to resident care staff in the assigned unit and assist them as needed.
- Check rooms routinely to ensure residents are being provided the care requested.
- Ensure communication with the resident's family in the event of the resident's transfer to a hospital or post-acute facility.
- Observe and uphold all departmental, community, and organizational safety standards and regulations.
- May complete minor housekeeping duties around the nursing unit.
- Complete other job tasks or job duties as assigned by the Assistant Director of Nursing (ADON) and/or the Director of Nursing (DON).
Qualifications & Experience:
- Must have a current, active, unencumbered license as a Licensed Vocational Nurse in the state of Texas.
- Minimum of 1 years’ nursing experience, preferably within a skilled nursing and/or geriatric community.
- Must have proven leadership ability.
- A strong ability to organize, plan, and direct nursing services and nursing staff.
- Must be able to handle emergencies, give emergency care, and give emergency instructions.
- Proficiency with technology, including online charting and communications.
- A service-minded approach to all aspects of the job.
